口腔医学 2010年 2月 (第 30卷第2期) Stomatology，Vo1．30，No．2，Feb．2010 ·93 · 摆式矫治器结合直丝弓矫治器治疗 Ⅱ类错牙台的疗效分析 潘 爽，王春玲，刘东旭，韩 敏，董仁萍 [摘要] 目的 分析安氏Ⅱ类错殆患者经摆式矫治器结合直丝 弓矫治器治疗后的牙颌面结构变化，探讨此方法建立正常咬 合关系的机制。方法 选择牙源性安氏 Ⅱ类错殆患者24例，在治疗前 (T1期)、推磨牙向远中后( 期)、直丝弓矫治结束后 (T3期)，拍摄头颅定位侧位片。采用 SPSS12．0软件包进行配对 t检验。结果 ①T1期 一1、2期，上颌第一二磨牙均远中移 动，但伴随着上颌切牙唇倾 、第一前磨牙近中移动并倾斜、第一磨牙远中倾斜及下颌平面顺时针旋转等副作用。②T1期 一rI3 期，下颌综合长度 、前下面高和升支高度增加显著(P0．O1)。上颌第一前磨牙、第一磨牙的矢状向位置无显著性差异，切牙 唇倾2．9。(P0．05)。即使增强了支抗控制，固定矫治阶段上颌第一磨牙近中移动3．0mm(P0．01)。结论 摆式矫治器 可以有效的远中移动上颌磨牙，但是 I类磨牙关系的建立是颌骨差异性生长和牙齿移动的综合结果。 [关键词] 摆式矫治器；磨牙远中移动；II类错殆 [中图分类号] R783．5 [文献标识码] A [文章编号] 1003-9872(2010)02-0093-03 Posttreatmentevaluationofpendulum appliancefollowedbyfixedorthodontictreatment ⅣShuang，WANGChun—ling，HANMin，DONGRen-ping．(DepartmentofOrthodontics，LabofOralBiomedicine，ShandongUniversity SchoolofStomotology，Jinan250012，China) Abstract： 0bjective Toanalyzemolardistalizationwiththedendulumappliance，itseffectontheanchorageteeth，andtheoutcome atthecompletionoforthodontictreatment．Methods Lateralcephalogramsof24adolescents(13girls，11boys)，takenafterpretreat- ment，afterdistalization，andafterfixedorthodontictreatment，wereevaluated．Themeanageofthesubjectsatthetimeofthepretreat— mentradiographwas11．6years．Results Thependulum appliancedistalizedthemaxillarymolars，buttherewassignificantlossofan- chorage．Afterfixedorthodontictreatment，themaxillaryfirstpremolarsandfirstmolarswerereturnedtotheirpretreatmentanteroposteri— orpositions．Thus，atpostdistalization，therewas3．0 mm ofprotrusionofthemaxillary firstmolars，despitetheanchoragereinforce· ment．Conclusions Thedendulum appliancefollowedbyfixedorthodontictreatmentcorrectedtheClassⅡsagittalrelationship．espe— ciallyduetothedentoalveolarchangessecondarytothespontaneousmandibulargrowthintheanteriordirectionduringfixedappliance treatment． Keywords： dendulum appliance；molardistalization；ClassⅡsagittalrelationship
