对比外剥内扎术与吻合器痔上粘膜环形切除术（PPH）治疗重度痔疮患者的临床效果 　　摘要：目的：比较分析两种不同术式治疗重度痔疮患者的临床效果。 　　方法：根据纳入/排除标准共选取84例病例作为本次研究对象并随机分组；研究组采用吻合器痔上粘膜环形切除术治疗，对照组采用传统外剥内扎术处理；比较两组患者参加工作时间、疼痛持续时间、术后并发症发生率等。 　　结果：研究组手术时间平均（18.2±11.4）min、疼痛持续时间平均（3.4±1.0）d、住院时间平均（4.2±1.5）d、参加工作时间平均（12.3±4.8）d，均较对照组有明显缩短，均有统计学差异（P0.05）；两组患者术后并发症发生率依次为18.6%，46.3%，有统计学差异（P0.05）。 　　结论：吻合器痔上粘膜环形切除术治疗重度痔疮手术操作简便，患者术后疼痛感明显减轻，恢复较快，能够尽早参加工作，临床效果理想，具有较高的推广价值。 　　关键词：重度痔疮 手术疗法 吻合器痔上粘膜环形切除术 　　Doi：10.3969/j.issn.1671-8801.2014.11.008 　　Comparison Ligation outer peel and stapling hemorrhoid ligation ring mucosal resection （PPH） of the clinical effects in treatment of patients with severe hemorrhoids 　　Guan Zisheng 　　Abstract：Objective：To compare the clinical effects of two different surgical treatment of patients with severe hemorrhoids. 　　Methods：Selected in total of 84 cases according to the inclusion / exclusion criteria ， as in this study and were randomized； Study Group used hemorrhoid stapler mucosal resection ring， the control group was dealed with the traditional Ligation outer peel surgery； and compared two groups of patients to participate in working hours， duration of pain， postoperative complication rate，etc. 　　Result： Study Group was Significantly shorter compared with the control group in the mean operative time （18.2±11.4） min， mean duration of pain （3.4±1.0） d， the average length of stay （4.2±1.5） d， the average time to work （12.3±4.8） d， which had significant differences （P0.05）； Two groups of patients the incidence of postoperative complications were 18.6%， 46.3%， which have a significant difference （P0.05）. 　　Conclusion：Hemorrhoidal circular stapler on mucosal resection for treatment of severe hemorrhoids surgery is easy to operate， patients with significantly reduce postoperative pain， faster recovery， able to work as soon as possible， the ideal clinical results， it has the promotion of high value. 　　Keywords：Severe hemorrhoids Surgical therapy Hemorrhoid stapler on mucosal resection ring 　　【中图分类号】R
