腹腔镜胆囊切除术的麻醉处理经验探讨 　　doi：10.3969/j.issn.1007-614x.2014.3.10 　　摘 要 目的：总结腹腔镜胆囊切除术的麻醉处理经验，进一步提高腹腔镜胆囊切除术的麻醉效果。方法：收治腹腔镜胆囊切除术患者160例，随机分成对照组和观察组，各80例，观察组采用全麻联合硬膜外麻醉，对照组采用单纯全麻。结果：两组在麻醉前、麻醉起效后和气腹时，SBP、DBP和HR比较差异无统计学意义（P0.05）；在气腹10分钟和气腹30分钟时，两组比较差异有统计学意义（P0.05）；与对照组相比，观察组的麻醉起效快、苏醒快、疼痛评分明显下降，差异有统计学意义（P0.05）。结论：全麻联合硬膜外麻醉具有麻醉效果好、术后苏醒快等优点。 　　关键词 腹腔镜 胆囊切除术 全麻联合硬膜外麻醉 单纯全麻 　　To investigate the anesthesia management experience of laparoscopic cholecystectomy 　　Zhu Jun 　　The anesthesia department of the Peoples Hospital of Jinkouhe District，Leshan City，Sichang Province，614700 　　Abstract Objective：Summarize the the anesthesia management experience of laparoscopic cholecystectomy，further improve the anesthetic effect of laparoscopic cholecystectomy.Methods：To admit 160 cases of patients with laparoscopic cholecystectomy from February 2012 to September 2013，were randomly divided into control group and observation group， each 80 cases，the observation group was treated with general anesthesia combined with epidural anesthesia，the control group was treates with general anesthesia.Results：Patients in the two groups before anesthesia，after anesthesia and pneumoperitoneum，there was no significant difference in SBP，DBP and HR（P0.05）.In 30 minutes and 10 minutes of pneumoperitoneum，there were significant differences between two groups（P0.05）.Compared with the control group，the observation group anesthesia faster，faster recovery，pain score decreased significantly，the difference was significant（P0.05）.Conclusion：General anesthesia combined with epidural anesthesia has the advantages of good effect of anesthesia，faster postoperative recovery，is worth the clinical promotion. 　　Key words Laparoscopic；Cholecystectomy；General anesthesia combined with epidural anesthesia；General anesthesia 　　腹腔镜胆囊切除术（LC）是治疗胆囊疾病常用的手术方式[1]，由于手术创伤小、出血少等优点，逐步被患者接受。尽管手术比较简单，但是对麻醉的要求较高，为总结腹腔镜胆囊切除术的麻醉处理经验，进一步提高腹腔镜胆囊切除术的麻醉效果，2012年2月-2013年9月收治腹腔镜胆囊切除术患者160例，进行回顾性分析，现报告如下。 　　资料与方法 　　2012年2月-2013年9月收治腹腔镜胆囊切除术患者160例，男
