纤支镜在老年严重肺部感染合并肺不张中的临床应用价值 　　【摘要】 目的：探讨老年严重肺部感染合并肺不张患者应用纤维支气管镜（纤支镜）治疗的价值。方法：选择2012年1月-2016年2月于笔者所在医院接受治疗的严重肺部感染合并肺不张老年患者40例进行研究，将其随机分为两组，每组20例。对照组按照常规治疗处理，研究组予以常规治疗联合纤支镜治疗，对比两组临床效果以及治疗前后血气分析指标（PaO2、PaCO2、SaO2、pH）。结果：研究组总有效率为95.00%，显著高于对照组的70.00%，比较差异有统计学意义（P0.05），治疗后研究组的PaO2、PaCO2、SaO2均优于对照组，比较差异均有统计学意义（P0.05）。结论：老年严重肺部感染合并肺不张采取纤支镜治疗可显著提高疗效，且更好地改善血气分析指标，值得借鉴。 　　【关键词】 老年； 严重肺部感染； 肺不张； 纤支镜 　　中图分类号 R563.4 文献标识码 B 文章编号 1674-6805（2016）24-0050-02 　　【Abstract】 Objective：To investigate the application value of fiberoptic bronchoscope（bronchoscopy） in the treatment of elderly patients with severe pulmonary infection combined with atelectasis.Method：Forty cases of severe pulmonary infection combined with atelectasis elderly patients admitted to our hospital from January 2012 to February 2016 were selected as the objects and randomly divided into two groups，each group of 20 cases.The control group was treated with conventional therapy，while the study group was given bronchoscopy treatment combined with conventional therapy，the clinical results，as well as the blood gas analysis（PaO2，PaCO2，SaO2，pH） before and after treatment of the two groups were compared.Result：The total effective rate of the study group was 95.00%，which was significantly higher than 70.00% of the control group，the difference was statistically significant（P0.05），after the treatment，the PaO2，PaCO2，and SaO2 of the study group were better than the control group，the differences were statistically significant（P0.05）.Conclusion：Bronchoscopy treatment can significantly improve the efficacy，and improve the blood gas analysis of elderly patients with severe pulmonary infection combined with atelectasis，it is worthy of reference. 　　【Key words】 Elderly； Severe pulmonary infection； Atelectasis； Bronchoscopy 　　First-author’s address：Fujian Province Geriatric Hospital，Fuzhou 350003，China 　　doi：10.14033/j.cnki.cfmr.2016.24.027 　　严重肺部感染属于老年人群常见疾病，在临床工作中这类患者极易合并肺不张。老年严重肺部感染合并肺不张患者因有效通气容积迅速降低，导致通气功能不足，机体电解质紊乱，严重情况下可致
