血清PCT定量检测对血培养结果的预测价值分析 　　[摘要]目的 探讨血清PCT定量检测对血培养结果的预测结果。方法 回顾性分析2014年1月～2015年12月来我院就诊同时进行血培养与PCT检测、且检测结果有效的500例患儿的临床资料。统计分析500例患儿的血培养与血清PCT检测结果，研究分析PCT在不同分界值对血培养的灵敏度、特异度、阳性预测值、阴性预测值。结果500名患儿血培养结果中有125例阳性患儿，包括113例细菌性感染和12例真菌性感染，有375例阴性患儿；125例阳性患儿在PCT检测中显示为阳性的有108例，包括细菌性感染97例，真菌性感染11例，阳性预测值为86.40%，阴性预测值为43.47%；以0.05 μg/L为分界值时，PCT对血培养的灵敏度为70.25%，特异度为56.67%，阳性预测值为28.55%，阴性预测值为91.56%。结论 血清PCT定量检测可以辅助迅速诊断患儿的血流感染，若能结合临床状况的观察分析，更能提高其检出率。 　　[关键词]血清PCT；定量检测；血培养结果；预测结果 　　[中图分类号] R446.11+1 [文献标识码] A [文章编号] 1674-4721（2016）09（b）-0146-03 　　[Abstract]Objective To study the predictive results of blood culture results by quantitative detection on serum PCT.Methods From January 2014 to December 2015，the clinical data of 500 patients for whom were treated in our hospital，at the same time the blood culture and quantitative detection of serum PCT were tested.The clinical data of patients with PCT were detected and the results were effective and retrospectively studied.The results of blood culture and serum PCT in 500 children were tested and analyzed.The sensitivity，specificity，positive predictive value and negative predictive value of PCT in blood culture were analyzed.Results 500 children with blood culture results in 125 cases with positive，including 113 cases of bacterial infection and 12 cases of fungal infection，375 cases with negative；125 children with positive in PCT detection were showed 108 cases positive，including 97 cases of bacterial infection and 11 cases of fungal infection，the positive predictive value was 86.4%，the negative predictive value was 43.47%；using 0.05 μg/L as the cut-off value，the sensitivity of PCT on blood culture with 70.25%，specificity with 56.67%，positive predictive value with 28.55%，negative predictive value with 91.56%.Conclusion The quantitative detection of serum PCT can help to diagnose the bloodstream infection in children quickly.If it can be combined with clinical observation and analysis，the detection rate will improve. 　　[Key words]Serum
