Anessentialcytokinesystemfortheosteoclastbiologyinmultiplemyeloma(MM)consistsofthereceptorofactivatorofNF-κB ligand(RANKL),itsreceptor(RANK),andthesolubledecoyreceptor,osteoprotegerin(OPG).Myelomacellscauseimbalance inOPG/RANKLinteractions.WemeasuredserumlevelsofOPG,soluble(s)RANKL,sRANKL/OPGratio,markersofdisease activity[LDH,CRP,interleukin-6(IL-6),β2-microglobulin(B2M)],andangiogenicfactors[hepatocytegrowthfactor(HGF), vascularendothelialgrowthfactor(VEGF)],in54newlydiagnosedMMpatientsandin25oftheminplateauphase.Alltheabove valueswerehigherinMMpatientscomparedtocontrolsanddecreasedinplateauphase.sRANKLandRANKL/OPGwerehigher withadvancingdiseasestageandskeletalgrade.Signi?cantcorrelationswerefoundamongRANKLandRANKL/OPGwithHGF, LDH,VEGF,IL-6,andB2M.Inconclusion,RANKLandOPGplaysigni?cantrolesinMMpathophysiology,asregulatorsofbone turnoverandmediatorsofangiogenesis. 1.Introduction the enhancement in osteoblast-induced bone resorption, while OPG blocks RANKL and inhibit
