Stromalinvasion(invasivegrowthoftumortissueintoportaltractsand?broussepta)isnowrecognizedasthemostimportant ?nding in the diagnosis of the well-di?erentiated type of early hepatocellular carcinomas (HCCs). In di?erentiating stromal invasionfrompseudoinvasion(benignhepatictissuein?brousstroma),thefollowing5itemsareuseful:(1)macroscopicor panoramicviewsofthehistologicalspecimen,(2)theamountof?brouscomponentsofstroma,(3)destructionofthestructure ofportaltracts,(4)lossofreticulin?bersaroundcancercells,and(5)cytokeratin7immunostainingforductularproliferation. Knowledgeofstromalinvasionisalsousefulforabetterunderstandingofthevasculature(hypovascularHCCs)andhistological features(fattychange)ofearlyHCCs.Invasionofpreexistingarteriesandportalveinscauseshypo-vascularityofHCCs.Further, hypovascularitycausesfattychangeasahypoxicchangeofcancertissues. 1.Introduction ?brous septa, portal tracts, and/or blood vessels [2–7].
