TheANCAconsensusprescribesscreeningbyindirectimmuno?uorescenceonneutrophils.Weevaluatedthe?rstautomated ANCA-patternrecognitionsystem.C-ANCA(n =39)andP-ANCA(n =40)sampleswereselectedfrompatientswithANCA- associated vasculitis (AAV). Non-AAV controls included sera from healthy controls (n = 40), sera with possible interfering antibodies(n=46),ormiscellaneousANCAreactivity(n=31).ANCAslideswereanalysedbyAKLIDESandroutine?uorescence microscopy.TheC-ANCApatternwasrecognizedbyroutinemicroscopyin92%and97%onethanol-andformalin-?xedslides, respectively. AKLIDES reported C-ANCA in 74% and 95%, respectively. P-ANCA was recognized by routine microscopy on ethanol-?xedneutrophilsin90%,whileAKLIDESreportedP-ANCAin80%.Typically,only65%and33%ofthesesamplesshowed theexpectedC-ANCAonformalin-?xedneutrophilsbyroutinemicroscopyandAKLIDES,respectively.AC-orP-ANCApattern wasobservedonethanol-?xedneutrophilsin28%and23%ofthecontrolsbyroutinemicroscopyandAKLIDES,respectively.Only 5%ofthecontrolsrevealedC-ANCAonformalin-?xedneutrophilsbyroutinemicroscopyandAKLIDES.Altogether,automated ANCA-patternrecognitionbyAKLIDESispromising.DistinctionofC-andP-ANCAisgood,butsensitivityonethanol-?xed neutrophilsneedsimprovement.Whenoptimized,patte
