Available evidence suggests that the use of CO2 insu?ation in endoscopy is more comfortable for the patient. The safety of CO2useincolonoscopyremainscontentious,particularlyinsedatedpatients.Theobjectiveofthepresentprospectivetrialwas to assess the safety of CO2 colonoscopies. Methods. 109 patients from our previous randomized CO2 colonoscopy study and anadditional238subsequentconsecutiveunselectedpatientswhohadaroutinecolonoscopyperformedinaprivatepractice wereenrolledfromApril2008throughSeptember2008.Allbut2patientsweresedated.Allpatientswereroutinelymonitored withtranscutaneousCO2 measurement.VolumesofCO2 administeredwerecorrelatedwithcapnographicmeasurementsfrom transcutaneousmonitoring.Results.Ofthe347patientsexamined,57%werewomen;mean(SD)ageofparticipantswasof60.2 years(12.8).Meanpropofoldosagewas136mg(64mg).MeanCO2valueswere34.7mmHg(5.3)atbaseline,38.9mmHg(5.5) uponreachingtheileum,and36.9mmHg(5.0)atexamination’send.MeanmaximumincreaseofCO2 was4.5mmHg(3.6).
