Materialandmethods.Anextendedlefthemihepatectomywasperformedin25pigs,andin14afterperformingaportosystemic shunt.Duringfollowup,liverregenerationwasestimatedbymacroscopicmarkerssuchaslivervolumeandsizeoftheportal?elds 2 2 [mm ]aswellastheamountofhepatocytesperportal?eldandtheamountofhepatocytespermm .Results.Regardlessofthe operationprocedure,thevolumeoftheremnantliverincreasedabout2.5foldattheendofthe?rstweekafterresection.Thesize oftheportal?eldsincreasedsigni?cantlyaswellasthenumberofhepatocytesintheportal?elds.Interestingly,thenumberof 2 hepatocytespermm remainedthesame.Conclusion.Afterextendedresection,liverregenerationwasachievedbyanextensive andsigni?canthyperplasiaofhepatocyteswithinthepreexistingportal?eldsandnotbydenovosynthesisofnewportal?elds. However,therewasnodi?erenceinliverregenerationregardingtheoperationprocedureperformedwithorwithoutportosystemic shunt.
