To determine whether carefully conducted health centre surveys canbe reliablemeansofmonitoringareaspecificmalariaepidemiology,wehavecomparedmalariaspecificindicesobtainedfrom surveysinhealthcentreswithindicesobtainedfromcross-sectionalsurveysconductedintheircatchmentcommunities. Methods:Aseriesofagestratified,seasonal,cross-sectionalsurveyswereconductedduringthepeakmalariatransmission seasonin2008andduringthefollowingdryseasonin2009insixecologicallydiverseareasinTheGambia.Participantswere patientswhoattendedthehealthcentresplusarepresentativesamplefromthecatchmentvillagesofthesehealthfacilities. Parasitaemia,anaemia,attributableproportionoffeverandanti-MSP1-19 antibody seroprevalencewerecomparedinthe healthfacilityattendeesandcommunityparticipants. Results:Atotalof16,230subjectscompletedthestudy;approximatelyhalfparticipatedinthehealthcentresurveysand halfinthewetseasonsurveys.Datafromboththehealthcentreandcommunitysurveysshowedthatmalariaendemicityin TheGambiaisnowlow,heterogeneousandseasonal.Inthewetseason,parasitaemia,seroprevalenceandfeverprevalence werehigherinsubjectsseeninthehealthcentresthaninthecommunitysurveys.Agepatternsofparasitaemia,attributable proportionsoffeverandseroprevalenceratesweresimilarinsubjectswhoparticipatedinthecommunityandhealthcentre surveys.
