ProtectiveEffectofHepatitisBVaccineCombinedwith Two-DoseHepatitisBImmunoglobulinonInfantsBorn toHBsAg-PositiveMothers HuaibinZou1,YuChen1*,ZhongpingDuan1*,HuaZhang2 1ArtificialLiverCenter,BeijingYouAnHospital,CapitalMedicalUniversity,Beijing,China,2DepartmentofObstetricsandGynecology,BeijingYouAnHospital,Capital MedicalUniversity,Beijing,China Abstract Background:DespitetheuseofhepatitisB(HB)vaccineandhepatitisBimmunoglobulin(HBIG),aportionofinfantsarestill non-orlow-responders,orevenimmunoprophylaxisfailure.Weaimedtodeterminetheimmuneresponseintheinfants fromthemothersbeingpositiveforhepatitisBsurfaceantigen(HBsAg),bywhichtheinfantsreceivedthreedosesofHB vaccineincombinationwithtwo-dose200IUHBIGinjections. Methods:Inthisretrospectivestudy,621infantsfromHBsAg-positivemothersinBeijingYouAnHospitalbetweenJanuary 2008andDecember2009wereincluded.Alltheinfantsweregiventhreedosesof10mgHBvaccine(at0,1and6monthsof age)andtwo-doseof200IUHBIG(atbirthandin2weeksofage).SerumHBsAgandantibodytoHBsAg(anti-HBs)inallthe infantsweredeterminedat7monthsofage. Results:Ofthe621infants,2.9%wereimmunoprophylaxisfailure(positiveforHBsAg),1.4%werenon-responders(anti-HBs undetectable),95.7%wereresponders.The594responderscouldbecategorizedintothreesubsets,22were10to99IU/L foranti-HBslevels,191were100to999IU/L,and381were$1000IU/L.Theimmunoprophylaxisfailureratewasat0%and 5.2%fortheinfantsofHBeAg-negativeandHBeAg-positivemothers(P,0.001).InfantsfrommotherswithdetectableHBV DNAhadhigherincidenceofimmunoprophylaxisfailurethanthoseofmotherswithoutdetectableHBVDNA(P=0.002). Thefactorsincludinggender,birthweight,gestationweeks,theratesofmaternalHBeAg-positive,anddetectableHBVDNA didnotcontributetothenoresponsetoHBvaccination. Conclusions:ThroughvaccinationbythreedosesofHBandtwo-doseofHBIG,majorityoftheinfants(95.7%)achieveda protectivelevelofanti-HBsat7monthsofage.MaternalHBeAg-positiveandHBVDNAdetectablewereassociatedwiththe immunoprophylaxisfailure,butnotcontributetothenon-orlow-responsetoHBvacci
