Methods:TheprimarydatabaseusedwastheTaiwanNationalHealthInsuranceResearchDatabase.Subjectsolder than20yearswithoutESRD,coronaryheartdisease,orstrokewereincludedinthestudy.Thecasedefinitionof goutinthepresentstudywasgoutdiagnosisandmedicaltreatmentforgout.AnESRDcasewasdefinedbythe presenceofchronicrenalfailurenecessitatinglong-termrenalreplacementtherapy.MultivariateCoxproportional hazardsmodelswereusedtoevaluatetheriskofESRDamonggoutpatients. Results:Theanalysisincludeddataof656,108patientswhowerefollowedupforameanof8.0years.Among them,19,963(3.0%)patientshadgout.Attheendof2008,2,377individuals(gout,n=276;non-gout,n=2,101) hadESRD,and861individuals(gout,n=77,27.9%;non-gout,n=521,24.8%)diedduetoESRD.Theratesof incidenceofESRDwere1.73and0.41casesper1,000patient-yearsinthegoutandnon-goutgroups.After adjustmentforage,sex,andhistoryofdiabetesmellitusand/orhypertension,goutwasassociatedwithahazard ratio(HR)of1.57forESRD(95%confidenceinterval[CI],1.38-1.79;P0.001).InpatientswithESRD,theadjusted HRfordeathinpatientswithgoutwas0.95(0.74-1.23,P=0.71),whichwassimilartotheHRobtainedinpatients withoutgout.
