DeLucaetal.BMCPediatrics2011,11:101 /1471-2431/11/101(8November2011) DeLucaetal.BMCPediatrics2011,11:101 /1471-2431/11/101 STUDY PROTOCOL OpenAccess SecretoryphospholipaseA2pathwayinvarious typesoflunginjuryinneonatesandinfants:a multicentretranslationalstudy DanieleDeLuca1,2*,EttoreCapoluongo1andVincentRigo3 ,for theStudygrouponSecretoryPhospholipaseinPaediatrics(SSPP) Abstract Background:SecretoryphospholipaseA2(sPLA2)isagroupofenzymesinvolvedinlungtissueinflammationand surfactantcatabolism.sPLA2playsaroleinadultsaffectedbyacutelunginjuryandseemsapromisingtherapeutic target.Preliminarydataallowforeseeingtheimportanceofsuchenzymeinsomecriticalrespiratorydiseasesin neonatesandinfants,aswell.OurstudyaimistoclarifytheroleofsPLA2anditsmodulatorsinthepathogenesis andclinicalseverityofhyalinemembranedisease,infectionrelatedrespiratoryfailure,meconiumaspiration syndromeandacuterespiratorydistresssyndrome.sPLA2geneswillalsobesequencedandpossiblegenetic involvementwillbeanalysed. Methods/Design:Multicentre,international,translationalstudy,includingseveralpaediatricandneonatalintensive careunitsandonecoordinatinglaboratory.Babiesaffectedbytheabovementionedconditionswillbeenrolled: broncho-alveolarlavagefluid,serumandwholebloodwillbeobtainedatdefinitetime-pointsduringthedisease course.Severalclinical,respiratoryandoutcomedatawillberecorded.Laboratoryresearcherswhoperformthe benchpartofthestudywillbeblindedtotheclinicaldata. Discussion:Thisstudy,thankstoitsmulticenterdesign,willclarifytherole(s)ofsPLA2anditspathwayinthese diseases:sPLA2mightbethecrossroadbetweeninflammationandsurfactantdysfunction.Thismayrepresenta crucialtargetfornewanti-inflammatorytherapiesbutalsoanovelapproachtoprotectsurfactantorspareit, improvingalveolarstability,lungmechanicsandgasexchange.
