Methods:Weperformedanarrativereviewofthemedicalliteraturetoidentifytheincidence,symptoms,and treatmentofhypophosphatemiaincriticallyillpatients.Specifically,wesearchedforanswerstothequestions whethercorrectionofhypophosphatemiaisassociatedwithimprovedoutcome,andwhetheracertaintreatment strategyissuperior. Results:Incidence:hypophosphatemiaisfrequentlyencounteredintheintensivecareunit;andcriticallyillpatients areatincreasedriskfordevelopinghypophosphatemiaduetothepresenceofmultiplecausalfactors.Symptoms: hypophosphatemiamayleadtoamultitudeofsymptoms,includingcardiacandrespiratoryfailure.Treatment: hypophosphatemiaisgenerallycorrectedwhenitissymptomaticorsevere.However,althoughmultiplestudies confirmtheefficacyandsafetyofintravenousphosphateadministration,itremainsuncertainwhenandhowto correcthypophosphatemia.Outcome:insomestudies,hypophosphatemiawasassociatedwithhighermortality;a paucityofrandomizedcontrolledevidenceexistsforwhethercorrectionofhypophosphatemiaimprovesthe outcomeincriticallyillpatients. Conclusions:Additionalstudiesaddressingthecurrentapproachtohypophosphatemiaincriticallyillpatientsare required.Studiesshouldfocusontheassociationbetweenhypophosphatemiaandmorbidityand/ormortality,as wellastheeffectofcorrectionofthiselectrolytedisorder.
