Methods:Weperformedapost-hocanalysisoftwodifferentprospectivestudiestoinvestigatethediagnosticand prognosticvalueofABGAparametersinpatientspresentingtotheEDwithacutedyspnea. Results:Weenrolled530patients(medianage74years).ABGAparameterswereneitherusefultodistinguish betweenpatientswithpulmonarydisordersandothercausesofdyspneanortoidentifyspecificdisorders responsiblefordyspnea.Onlyinpatientswithhyperventilationfromanxietydisorder,thediagnosticaccuracyofpH andhypoxemiarenderedvaluablewithanareaunderthereceiveroperatingcharacteristicscurve(AUC)of0.86. PatientsinthelowestpHtertilemoreoftenrequiredadmissiontointensivecareunit(28%vs12%inthefirst tertile,P0.001)andhadhigherin-hospital(14%vs5%,P=0.003)and30-daymortality(17%vs7%,P=0.002). Cumulativemortalityratewashigherinthefirst(37%),thaninthesecond(28%),andthethirdtertile(23%,P= 0.005)during12monthsfollow-up.pHatpresentationwasanindependentpredictorof12-monthmortalityin multivariableCoxproportionalhazardanalysisbothforpatientswithpulmonary(P=0.043)andnon-pulmonary disorders(P=0.038). Conclusions:ABGAparametersprovidelimiteddiagnosticvalueinpatientswithacutedyspnea,butpHisan independentpredictorof12monthsmortality.
