Abstract ABSTRACT Objective： To research the effects of lappaconite hydrobromide injection on inflammatory cytokines and myocardial enzyme during operation, which tourniquet produced low limb ischemia reperfusion. Methods ： Forty patients who undergoing unilateral knee-joint, ankle or near ankle surgery, ASA I-II, were randomly divided into two groups, 20 patients in lappaconite hydrobromide group( Group L) and another 20 patients in control group( Group C). Group L: intravenous injected lappaconite hydrobromide 8 mg slowly before low limb tourniquet inflation, and tourniquet deflated after 60-70 minutes. Group C: intravenous injected normal saline 4ml slowly. 2-3 ml blood samples were taken from the patients to detected TNF-α, IL-1β, CK concentration before tourniquet inflation(T ), 10 min(T ) and 30 min after tourniquet deflation(T ). Record NIBP, 0 1 2 SpO and HR at before tourniquet inflation(t ), before tourniquet deflation, tourniquet 2 0 deflation, 5 min after tourniquet deflation (t ), 20 min after tourniquet deflation (t ). 1 2 Results ： The systolic pressure(SP) and diastolic blood pressure(DP) at the time before tourniquet deflation significantly increased than t0 in two groups (P＜0.05). SP and DP significantly decreased at tourniquet deflation than before tourniquet deflation in Group C (P＜0.05). SP,DP and HR were lower in Group L than Group C before tourniquet deflation and tourniquet
