Methods:Patients≥18yearsofageadmittedtotheemergencywardforsymptomsofpsychosiswere consecutivelyrandomizedtorisperidone(n=53),olanzapine(n=52),quetiapine(n=50),orziprasidone(n=58), andfollowedforupto2years. Results:Atotalof213patientswereincluded,ofwhich68%weremales.Thesamplerepresentedadiverse populationsufferingfrompsychosis.AtadmittancethemeanPositiveandNegativeSyndromeScale(PANSS)total scorewas74pointsand44%wereantipsychoticdrugna?ve.Theprimaryintention-to-treatanalysesrevealedno substantialdifferencesbetweenthedrugsregardingthetimesuntildiscontinuationofinitialdrug,untildischarge fromindexadmission,oruntilreadmission.Quetiapinewassuperiortorisperidoneandolanzapineinreducingthe PANSStotalscoreandthepositivesubscore.QuetiapinewassuperiortotheotherdrugsindecreasingthePANSS generalpsychopathologysubscore;indecreasingtheClinicalGlobalImpression-SeverityofIllnessscalescore (CGI-S);andinincreasingtheGlobalAssessmentofFunctioning-Splitversion,Functionsscalescore(GAF-F). ZiprasidonewassuperiortorisperidoneindecreasingthePANSSpositivesymptomssubscoreandtheCGI-Sscore, andinincreasingtheGAF-Fscore.Thedrugsperformedequallywithregardstomosttolerabilityoutcomesexcept ahigherincreaseofhip-circumferenceperdayforolanzapinecomparedtorisperidone,andmoregalactorrhoea forrisperidonecomparedtotheothergroups.
