《中国医学创新》第l4卷第1期(总第391期)2017年1月 论 著 Lunzhu 无创呼吸机辅助通气治疗COPD致肺源性 心脏病伴Ⅱ型呼吸衰竭患者的临床研究术 付会恒 田亚莉④ 李丽荣④ 巫剑峰 曾祥富 【摘要 】 目的：探讨无创呼吸机辅助通气治疗慢性阻塞性肺疾病 (COPD)致肺源性心脏病伴Ⅱ型呼 吸衰竭的临床效果。方法：选取本院收治的COPD致肺源性心脏病伴Ⅱ型呼吸衰竭患者 6O例作为研究对 象，利用随机数字表法将所有研究对象分为对照组和观察组，每组各 3O例。对照组患者采用常规治疗， 观察组患者实施无创呼吸机辅助通气治疗，观察两组患者的治疗效果。结果：观察组呼吸困难、胸闷心 悸、剧烈咳嗽缓解时间均明显短于对照组 ，差异均有统计学意义 (P0．05)；观察组血氧饱和度 (SaO)、 酸碱值 (pH)、动脉血氧分压 (PaO：)、二氧化碳分压 (PaCO：)均明显优于对照组 ，差异均有统计学意 义 (PO．05)；两组患者治疗总有效率比较，差异有统计学意义 (PO．05)。结论：无创呼吸机辅助通气 治疗COPD致肺源性心脏病伴Ⅱ型呼吸衰竭患者的临床效果突出，值得推广。 【关键词】无创呼吸机辅助通气； COPD； 肺源性心脏病； 11型呼吸衰竭 TheCfinicai ResearchofNoninvasiveVentilatorAssistedVentilationin theTreatmentofPulmonary HeartDiseasewithType IIRespiratoryFailureCausedbyCOPD／FU Hui—heng，TIAN Ya—li，LILi— rong．etaidM／edicalInnovationofChina，2017，14(01)：009-012 【Abstract】Objective：Toinvestigatetheclinicaleffectofnoninvasiveventilatorassistedventilationinthe treatmentofpulmonaryheartdiseasewithtypeI／respiratoryfailurecausedbyCOPD．Method：60patientswith pulmonaryheartdiseaseassociatedoftypeI1respiratoryfailureinourhospitalwereselectedastheresearchobjects． Accordingtotherandom numbertablemethod，theyweredividedintocontrolgroupandobservationgroup，30 casesineachgroup．Thecontrolgroupwas treatedwithconventionaltreatment，theobservationgroupwasrteated withnoninvasiveventilation，theclinicaleffectsoftworgoupswereobserved．Result：Thebreathingdifficulties， chesttightness，heartpalpitations，acutecoughrelieftimeoftheobservationrgoupweresignificnatlyshorterthna thoseincontrolgroup，thedifferenceswerestatisticallysingificant(P0．05)．TheSaO2，pH，PaO2，PaCO2 oftheobservationgroupweresignificantlybetterthanthoseincontrolgroup，thedifferenceswerestatistically singificant(PO．05)．Conclusion：Noninvasiveventilatorassistedventilationinthetreatmentofpulmonaryheart diseasewihttypeIIrespiratoryfailurecausedbyCOPDobviouslyhaveclinicaleffect，itisworthofpromoting． 【Keywords】Noninvasiv
