■ 新医学导刊 2008年1月第7卷第1期 ’．， ， cP ●论 著● 骨外固定架治疗下肢骨折不愈合4例临床分析 朱志忠 崇州市第二人民医院(四川 崇州 61 1230) 中图分类号：R683．42 文献标识码：A 文章编号：1672-3783(2008)一1-0001-02 【摘 要】目的 了解骨外固定架治疗下肢骨折不愈合的情况。方法 通过对4例不愈患者进行分析、治疗。股骨病例采用的手术 方式：进行原有内固定取出、对不愈合断端修整、加压，对股骨行转子下2CM截骨并作延长用，用单臂可延长外固定支架，三组固定共 七颗钉，近端二组钉，远端一组钉，远端钉呈品字排列。胫骨病例手术方式：胫骨行胫骨结下3CM处结骨，行半环槽式外固定支架。 对骨髓炎病例在术中进行严格的清创，术后进行伤口灌洗3天。结果 外固定支架治疗骨折不愈是一种比较实用和理想的固定器 材。 【关键词】外固定架 下肢骨折 不愈合 Clinical Analysis on the Outcome of External Skeletal Fixation in Treating Four Patients with Fracture of Lower Limb Disunion ZHU Zhi——zhong Chongzhou Second Peoples Hospital，Sichuan 611230，China 【Abstract】Objective To understand the situation of fracture of lower limb disunion treated with external skeletal fixation．Method To anyl— yse 4 patients with fracture of lower limb disunion．Femur cases adopt modus operandi。the methods ale as follows：take out of original internal fixation，trim the broken ends of fractured bone and compress the fragments of the fracture，subtrochanteric osteotomy and bone lengthening were applied in subtrochanteric fractures of the femur．Using sin~e arnl can prolonged external fixed stent．Three groups have seven nails，the proximal end include two groups and the distal end have one group．Th e methods that shin bone cases adopt are as follows：Th e stability of half — ring sulcated extemai fixator was叩plied in cases with shin bone．Debridement should be do for patients with osteomyelitis dufng operation ． Irrigation after operation was performed．Condusion The extemai fxation is a kind of comparative practical and ideal equipment． 【Key words】External fixation；Fracture of lower limb；Disunion 目前对下肢长骨骨折的内固定方式主要有钢板类与钢钉 1．2 治疗方法 对选择的4例病例在硬膜外麻醉下进行。股 类，其中以带锁髓内针为骨折治疗的首选方法，随着选择对内固
