8 hinese Journal ofClinical Practical Medicine, June 2008、Vol 9．No 6 · 论著· 高血压病的经颅多普勒分析 卓 嘎 徐建国 杨 静 【摘要】目的 通过经颅多普勒(TcD)检测，了解高血压病患者的脑血管主要动脉的血流动力学变化及各血流的生 理参数，有利于临床医生对高血压痛患者临床症状的病理变化作出分析及治疗提供依据，同时对高血压病的分期提供客 观的依据。方法 将已确诊的高血压病患者，与颞窗及枕窗通过低频脉冲多普勒(2MHz)，获得脑血管主要动脉的多普勒 超声频移信号，进行TCD检测。结果 TCD检测后了解高血压病患者的脑血管主要动脉，主要有以下特点：(1)较高的 异常率；(2)收缩期的血流速度增高(即脑血管痉挛)；(3)收缩期血流速度降低(即脑供血不足)；(4)频谱形态改变(即脑动脉 硬化)。结论 经TCD检测，能较灵敏的反映高血压病的脑血管的病理及血流动力学改变，且不同病期其表现各有特点， 故有利于临床医生更好地给予对症治疗，同时对高血压病的分期提供客观的依据。 【关键词】经颅多普勒 高血压病 Analysis on the Hypertension by Transcranial Doppler Sonograph~ Zhuo Ga，Xu Jianguo．Yang Jing．Department of Ultrasound，Lhasa General Hospital of PLA，Lhasa，850007 China [Abstract]Objective To evaluate the haemodynamics change and the physiological blood flow parameter of the main cerebral vascular in the patients suffered from hypertention，which is make for the clinician to evaluate the pathological change of clinical symptom in patients suffered from hypertension and to provide the therapeutic basis，at the same time it can provide the objective staging basis to the hypertension．Methods The definite patients suffered from hypertension were undergone low frequency pulse doppler examination(2MHz)of transtemporal window and foramen magnum window and the doppler ultrasound frequency shift signal of the main cerebral vascular was obtained then analyzed by the method of TCD．Results The abnormalities observed in the main cerebral vascular of patients suffered from hypertension after TCD examination include：(1)a much higher abnormality rate．(2)increase of blood velocity in the systolic phase(cerebral vasospasm)．(3)decrease of blood velocity in the systolic phase(cerebral ischemia)．(4)morphous change of frequency spectrum(cerebral arteriosclerosis)． Conclusion The pathological and haemodynamics changes of cerebral vascul
