脐带脱垂及脐带先露23例临床研究[摘要] 目的 探讨脐带脱垂与脐带先露的高危因素、诊断方法、临床处理及围生儿预后。方法 回顾性分析1998年3月～2008年9月23例脐带脱垂和脐带先露病例。结果 8例脐带脱垂,立即行剖宫产者3例,获活婴3例;臀助产者3例,获活婴2例;死胎及死产各一例经阴道分娩。15例脐带先露,均行剖宫产术,获活婴15例。结论 注意脐带脱垂的高危因素,及早发现,及时处理,可减少围生儿死亡率。 [关键词] 脐带脱垂;脐带先露;高危因素;分娩方式;围生儿 [中图分类号] R714.56 [中图分类号] A[文章编号] 1673-9701(2009)24-115-02 Clinical Analysis of 23 Cases of Prolapse or Presentation of Umbilical Cord YU Ying Department of Obstetrics and Gynecology,The First People’s Hospital of Changshu,Jiangsu 215500,China [Abstract] Objective Discusses prolapse or presentation of umbilical cord the high-risk factor,the diagnosis method,clinical processing and perinatal prognosis.Methods 23 cases of prolapse or presentation of umbilical cord from March 1998 to September 2008 were analyzed retrospectivel.Results 8 example prolapse of umbilical cord,cesarean section 3 examples immediately,attains the live infant 3 examples; The vaginal delivery 3 examples,attain the live infant 2 examples; fetal death and stillbirth each example after vaginal delivery. 15 example presentation of umbilical cord,caesarean section 15 examples,attains the live infant 15 examples.Conclusion Pay attention to high-risk factor which prolapse of umbilical cord,discovered early,prompt processing,may reduce perinatal mortality. [Key Words] Prolapse of umbilical cord;Presentation of umbilical cord;High-risk factor;Way of delivery;Perinatal infant 胎膜未破时脐带位于胎先露部前方或一侧称为脐带先露,也称隐性脐带脱垂。胎膜破裂后,脐带脱出于宫颈口外,降至阴道甚至外阴,称为脐带脱垂[1]。脐带脱垂为一种极度危险的分娩并发症,发生突然,难以预测,发生率各家报道差异很大,在0.4%～10%之间,一旦发生,围产儿致死率可达20%～30%[2],尤以脐带脱垂危害最大。现将我院1998年3月～2008年9月发生的脐带脱垂和脐带先露处理总结如下。 1临床资料 1.1一般资料 1998 年3月～2008年9月我院共发生脐带脱垂和脐带先露23例,其中8例为脐带脱垂,15例为脐带先露。孕妇年龄 21～38 岁;初产妇16例,经产妇7例;孕周32+5～41+2周,小于37周4例;头位16例,臀位6例,横位1例;脐带过长3例;存活婴儿20例,死亡3例(其中死胎1例)。 1.2诊断方法 (1)胎头高浮者自然破膜或人工破膜后突然胎心减慢,阴道检查时触及脐带;(2)临床直接观察到脐带脱垂至阴道外;(3)待产或试产过程中持续胎心电子监测出现重度变异减速或反复出现晚期减速于剖宫产术中确诊;(4)剖宫产术中发现脐带先露,之前无任何异常征象。 1.3处理及结果 15例脐带先露产妇,其中1例未进入产程前出现胎动减少,NST无反应型,4例产程中胎心监护发现变异减速及晚期减速,羊水胎粪污染,1例臀位足先露
