， ， · · J Clin Electrocardiol 2012 Apr.21．No.2 102 ·临床研究· P 波离散度对房颤消融术后预后的影响 常栋 褚振亮 高连君 杨延宗 【摘要】目的探讨P 波离散度（Pd）对阵发房颤环肺静脉消融（CPVA）术远期预后的预测价值。方法顺序 入选 116 例阵发性房颤患者行CPVA 术至肺静脉电隔离。测量和计算算末次消融术前和术后P 波最大值 （Pmax），P 波最小值（Pmin），P 波离散度（Pd）。结果 随访45.7±19.2 个月，112 例患者完成了研究，成功率达 78.6%。根据随访结果，分为成功组和复发组。两组消融术前Pmax、Pmin 和Pmax 相似。成功组术后Pmax 和 Pd 显著降低[Pmax（101.2±10.9）vs. （117.3±13.8）ms ，p 0.01 ；Pd （34.8±6.7）vs. （49.3±10.3）ms ，p 0.01]，而 复发组术后P 波各参数均无明显变化。两组相比，成功组术后Pd 和Pmax 较复发组显著降低[Pmax（116.4± 9.9）ms ，Pd （49.2±8.8）ms]。COX 回归分析显示两组左房内径和LVEF 均无明显差异；术后Pd 和Pmax 是阵 发房颤CPVA 术后远期复发的独立预测因子。结论Pmax 和Pd 反映了心房非均质性活动，术后Pmax 和Pd 可作为阵发房颤CPVA 术后远期复发的预测因素。 【关键词】心房颤动；环肺静脉消融术；P 波离散度 ［中图分类号］R541.7 R540.4+1 ［文献标识码］A ［文章编号］1005－0272 （2012）02－102-04 The effects of P wave dispersion on the long-term prognosis of paroxysmal atrial fibrilation after cricumferential pulmonary vein ablation Chang dong, Chu zhenliang, Gao lianjun, Yang yanzong.
