黄连四物汤治疗早期糖尿病肾病临床疗效观察及对血清AGEs水平影响摘 要：目的：观察黄连四物汤配合西医治疗对早期糖尿病肾病的临床疗效，及对血清AGEs水平的影响。方法：42例患者在西医治疗的基础上给予黄连四物汤，1剂/日，分2次服。分别在治疗前、治疗第3周末、第6周末、第9周末、第12周末测定空腹血糖（FBG）、血肌酐（SCr）、尿微量白蛋白排泄率（AER），比较各时间点上述指标差异；在治疗前、后测定血甘油三酯（TG）、总胆固醇（TC）、糖化血红蛋白（HbA【sub】1【/sub】C）水平及晚期糖基化终末产物（AGEs），比较治疗前、后差异。应用SPSS 13.0统计软件进行分析。结果：FBG在治疗第6周后下降显著（P＜0.01）；尿AER在治疗第6周后下降显著（P＜0.01）；SCr在治疗第9周后下降明显（P0.033）；治疗后HbA【sub】1【/sub】C、TG、TC、AGEs水平均较治疗前明显下降（P＜0.01）。结论：黄连四物汤配合西医治疗具有良好临床疗效，不仅能显著下调血糖及糖化血红蛋白，还能调节血脂，明显降低早期糖尿病患者尿AER；同时还降低血浆AGEs水平，这可能是其降低尿AER的机制之一。 关键词：早期糖尿病肾病；黄连四物汤；临床疗效；晚期糖基化终末产物 中图分类号：R587.1 文献标识码：B 文章编号：1673-7717（2011）04-0921-03 Clinical Effect of Huangliansiwu Formula in Treating Early Diabetic Nephropathy and Influence on Serum AGEs WEN Ping-fan, HUANG Gui-qiong （HuiZhou Hospital of Traditional Chinese Medicine, HuiZhou, GuangDong Province, 516001） Abstract:Objective：To observe the clinical effect of Huangliansiwu formula in treating early diabetic nephropathy combined with western medical therapy and its influence on serum serum advanced glycation end products （AGEs）. Methods：42 patient were given the western treatment basically, and also given the Huangliansiwu liquid medicine twice a day. Tested the fasting blood-glucose（FBG）,serum creatinine（SCr）, urinary albumin excretion rate（UAER） before therapy and on the third, sixth, ninth, twelfth weekends. Compared the difference of these indexes on different time points. Also compared the difference of triglycerides（TG）, total cholesterol（TC）, glycosylated hemoglobin（HbA1c）, AGEs before and after treatment. Results FBG and UAER had descended obviously from the sixth weekend, the SCr had descended visibly from the ninth weekend, the TC, TG, HbA1c, AGEs decline markedly after treat. Conclusion Huangliansiwu formula had good clinical effect to go with western treatment. It was not only could descend FBG,HbA1c and UAER, also could accommodate blood-fat. Meanwhile, it could descend serum AGEs. This may be one of the mechanism in degrading UAE
