巨刺阳陵泉穴对胆道系统影响 　　[摘 要]目的：探讨针刺阳陵泉穴对胆道系统运动的效果与机理。方法：将60例胆囊疾病患者随机分为左阳陵泉(左穴组)、右阳陵泉(右穴组)和双阳陵泉组(双穴组)，每组各20例，在B超下观察3组对胆囊收缩和胆总管扩张的影响。结果：左穴组、右穴组和双穴组对胆囊收缩和胆总管扩张均有明显作用(P＜0.05)，但3组间的作用差异无显着性意义(P＞0.05)。结论：左阳陵泉与右阳陵泉对胆道系统有同样的效应，故临床不宜只取右穴，而忽略左穴，也不必左右双穴同时取。 　　[主题词]巨刺；穴，阳陵泉；胆囊/针灸效应；胆总管/针灸效应 　　Effect of opposing needling at point Yanglingquan (GB 34) on bi liary tract system 　　Liu Guangting1， Wang Shumin2, Wang Lijian2(1.College of Acupuncture Massage, Shandong University of TCM, Jinan 250014, China; 2.A ffiliated Hospital of Shandong University of TCM) 　　ABSTRACT Objective To explore the effect of opposing needling at Yanglingquan (GB 34) on motion of the biliary tractsystem and mechanisms .Methods 60 cases with biliary tract diseases were randomly divi ded into left Yanglingquan group (left point group), right Yanglingquan group (right point group), and bilateral Yanglingquan group (Bilateral points group), 20 cases in each group. Effects on contraction of the gallbladder and dilation of the common bile duct were investigated by B-ultrasonography.Results All the 3 groups had significant action on contraction of the gallbladder and dilation of the common bile duct (Ｐ＜0.05)，with no significant difference among the 3 groups (Ｐ＞0.05). Conclusion Yanglingquan (GB 34) points on both sides have same effects on motion of the biliary tract system. 　　KEY WORDS Opposing Needling; Point GB 34 (Yanglingquan); Gallbladder/acup eff; Common Bile Duct/acup eff 　　 　　巨刺是针刺取穴法之一，源于《内经》。《灵枢?官针》：“巨刺者，左取右，右取左”。临床上治疗胆囊疾病，绝大多数医生只取右侧阳陵泉穴治疗，未曾见过只取左侧阳陵泉者。那幺左穴对胆囊是否有作用，左右两穴是否有差异，笔者以B超为客观指标，观察于针刺前后胆囊疾病患者胆囊和胆总管的运动，现报道如下。 　　 　　1 对象与方法 　　 　　1.1 观察对象及分组 　　60例胆囊疾病患者均??我院门诊及住院的经B超确诊的病例，以1∶1∶1分组比例，按针刺试验的顺序随机分为左阳陵泉(左穴组)、右阳陵泉(右穴组)和双阳陵泉(双穴组)3组。左穴组20例，男12例，女8例；年龄20～65岁，平均41.7岁；病程25天～8年，平均465.3天。单纯胆囊炎18例，胆囊结石合并胆囊炎2例。右穴组20例，男9例，女11例；年龄23～60岁，平均39 .85岁；病程21天～6.5年，平均420.6天。单纯胆囊炎18例，胆囊结石合并胆囊炎2例。双穴组20例，男10例，女10例；年龄25～68岁，平均46.12岁；病程36天～11年，平均498.7天。单纯胆囊炎17例，胆囊结石合并胆囊炎3例。3组年龄、病程、诊断及针刺前胆囊面积和胆总管内径等差异无显着
