52例糖尿病合并肺结核胸部X线及CT分析 　　[摘要] 目的 探讨52例糖尿病合并肺结核的胸部X线及CT分析。方法 回顾性分析该院收治的52例糖尿病合并肺结核患者临床资料，患者均进行胸部X线及CT检查，比较两种影像检查的结果差异，并分析其影像学表现。 结果 CT检查组空洞、小结节征及大片实变影的检出率分别为67.3%、51.9%、82.7%显著高于X线检查组46.2%、26.9%及61.5%，比较差异有统计学意义（P0.05）。结论 胸部CT及X线检查诊断糖尿病合并肺结核具有其特征性，临床应综合分析以提高其诊断敏感性，值得临床选择。 　　[关键词] 糖尿病；合并；肺结核；胸部X线；胸部CT 　　[中图分类号] R816 [文献标识码] A [文章编号] 1672-4062（2016）07（b）-0052-02 　　[Abstract] Objective To analyze the clinical application of chest X-ray and CT examination in 52 cases of diabetes complicated with pulmonary tuberculosis. Methods The clinical data about 52 cases of diabetes complicated with pulmonary tuberculosis in our hospital was retrospectively analyzed. The subjects received the chest X-ray and CT examination. The examination results and imaging manifestations were compared. Results Based on CT examination， the detection rate of cavity， tubercule and large areas of consolidation was 67.3%， 51.9% and 82.7%； based on chest X-ray examination， the rate was 46.2%， 26.9% and 61.5% （P0.05）. Conclusion The chest X-ray and CT examination is of great importance in the diagnosis of diabetes complicated with pulmonary tuberculosis. The comprehensive analysis can improve the sensitivity of diagnosis in clinical practices. 　　[Key words] Diabetes； Complicated； Pulmonary tuberculosis； Chest X-ray； CT examination 　　糖尿病是临床上常见的慢性内分泌疾病之一，糖尿病患者发生肺结核较正常者高4倍左右，其发病率呈逐年上升的趋势[1-2]，严重威胁患者生命安全及身心健康，同时糖尿病合并肺结核影像学表现往往无特异性表现，给临床诊断和治疗带来一定的挑战。为探讨52例糖尿病合并肺结核的胸部X线及CT分析。该研究回顾性分析该院2013年1月―2015年12月间收治的52例糖尿病合并肺结核的胸部X线及CT检查临床资料，现报道如下。 　　1 资料与方法 　　1.1 一般资料 　　回顾性分析该院收治的52例糖尿病合并肺结核患者临床资料。其中男性32例，女性20例；年龄39～72岁，平均（47.8±5.9）岁；糖尿病病程为1～23 年，平均（12.7±3.8）年；其中抗核抗体阳性23例，痰找结核抗菌阳性47例；患者均符合糖尿病合并肺结核的诊断标准[3]。 　　1.2 方法 　　患者均进行胸部CT及X线检查，患者采用美国GE公司生产的D6000 DR摄片机进行胸部正侧位片检查，摄片条件为电压120 kV，电流320 mA，采用荷兰飞利浦多排螺旋CT扫描机，扫描范围为肺尖至双侧肋膈角，扫面参数：电压120 kV，电流180 mA，层厚5 mm，进行胸部增强CT扫描37例； 　　1.3 观察情况 　　记录两种影像检查检出肺结核情况。 　　1.4 统计方法 　　采用SPSS 18.0软件分析观察项数据，计数资料用χ2检验表示。 　　2 结果 　　2.1 胸部X线、CT影像表现 　　52例糖尿病合并肺结核患者行X线检查，发现空洞24例（46.2%），空洞≥3
