PFNA与DHS治疗老年股骨粗隆间骨折临床研究 　　[摘要] 目的 比较PFNA与DHS治疗老年股骨粗隆间骨折的手术效果。 方法 73例股骨粗隆间骨折患者根据治疗方法不同分为PFNA组（n=38）与DHS组（n=35）。比较两组的髋关节功能，并观察记录两组的手术时间、术中出血量、骨折愈合时间。 结果 PFNA组与DHS组髋关节功能评定的优良率分别为92.1%（35/38）、80.0%（28/35），组间比较差异有统计学意义（χ2=4.237，P0.05）。PFNA组的手术时间（72.7±11.6）min，术中出血量（84.6±10.7）mL，骨折愈合时间（12.1±1.3）周，DHS组的手术时间（112.8±10.2）min，术中出血量（173.3±31.2）mL，骨折愈合时间（14.8±2.7）周，两组比较差异有统计学意义（P0.05或P0.01）。两组并发症发生率比较差异有统计学意义（P0.05）。 结论 PFNA与DHS均为治疗老年股骨粗隆间骨折的有效方法，但PFNA疗效显著优于DHS内固定，PFNA治疗老年股骨粗隆间骨折具有出血少、手术时间短、并发症少等优点。 　　[关键词] 老年股骨粗隆间骨折；PFNA；DHS 　　[中图分类号] R687.3 [文献标识码] B [文章编号] 1673-9701（2015）11-0062-03 　　[Abstract] Objective To compare operation effects of PFNA and DHS in treatment of femoral intertrochanteric fracture. Methods A total of 73 cases of intertrochanteric fracture patients were divided into PFNA group （n=38） and DHS group （n=35） according to the different treatment methods. The function of hip joint were compared between two groups and the operation time， the amount of bleeding and the time of fracture healing were observed and recorded. Results The excellent and good rate of PFNA group and DHS group was 92.1% （35/38） and 80.0% （28/35）， the difference was statistically significant between two groups （χ2=4.237， P0.05）. The operation time of group PFNA was（72.7±11.6）min， amount of bleeding during the operation was（84.6±10.7）mL， healing time of fracture was（12.1±1.3）weeks. The amount of bleeding operation time in DHS group was（112.8±10.2）min， amount of bleeding during the operation was（173.3±31.2）mL， healing time of fracture was（14.8±2.7）weeks， compared two groups， the difference was statistically significant （P0.05 or P0.01）. There was significant difference in complications between two groups（P0.05）. Conclusion PFNA and DHS are both effective ways in treatment of senile femoral intertrochanteric. PFNA in treatment of senile intertrochanteric fractures with less bleeding， short operation time， less complications， especially， the therapeutic effect is superior than that of DHS inte
