T3N1M0以下食管癌应用俯卧位和左侧卧位胸腔镜食管切除术疗效对比 　　[摘要]目的 比较临床分期T3N1M0以下食管癌应用俯卧位和左侧卧位胸腔镜食管切除术的治疗效果。方法 选择大连市中心医院2013年1月～2015年9月收治的临床分期T3N1M0以下的食管癌78例患者，根据随机数字表法分为A组（俯卧位胸腔镜食管切除术）及B组（左侧卧位胸腔镜食管切除术），每组39例。比较两组患者的手术相关指标、术后并发症发生率及预后指标。结果 A组的手术时间[（215.2±16.1）min ]明显短于B组[（286.8±32.6）min]，术中出血量[（278.5±21.5）ml ]明显少于B组[（357.7±33.0）ml]，清扫淋巴结数[（8.5±1.2）枚]明显多于B组[（6.2±0.8）枚]，差异均有统计学意义（P0.05）。结论 针对T3N1M0以下食管癌患者采取俯卧位及左侧卧位胸腔镜手术，两者的临床疗效相同，但俯卧位手术时间更短并对清扫淋巴结更有利。 　　[关键词]食管肿瘤；胸腔镜手术；俯卧位；左侧卧位 　　[中图分类号] R735.1 [文献标识码] A [文章编号] 1674-4721（2018）1（a）-0034-04 　　[Abstract]Objective To compare the treatment effect of prone position and left lateral position thoracoscopic esophagectomy applied in esophageal cancer under T3N1M0.Methods From January 2013 and September 2015，78 patients with esophageal cancer under T3N1M0 of clinical stages treated in Dalian Central Hospital were selected and they were divided into group A （prone position thoracoscopic esophagectomy） and group B （left lateral position thoracoscopic esophagectomy） according to the random number table method，and there were 39 cases in each group.Surgical related indexes，the incidence of postoperative complications and prognostic indicators in patients between the two groups were compared.Results The operation time of group A （[215.2±16.1] min） was obviously shorter than that of group B （[286.8±32.6] min），the amount of bleeding during operation of group A （[278.5±21.5] ml） was obviously fewer than that of group B （[357.7±33.0] ml），the number of lymph nodes of group A （[8.5±1.2]） was obviously more than that of group B （[6.2±0.8]），the differences were statistically significant （P0.05）.Conclusion For esophageal cancer under T3N1M0，surgical outcomes are similar between prone position and left lateral position thoracoscopic esophagectomy，but the operation time of prone position operation is shorter and it is more favorable for cleaning the lymph nodes. 　　[Key words]Esophageal neoplasms；Thoracoscopic surgery；Prone position；Left lateral position 　　目前?R床分
