专病一体化管理理念在血液透析合并低血压患者中的效果研究 　　[摘要] 目的 探讨专病一体化管理理念在血液透析合并低血压患者中的效果研究。 方法 选择2013年3月～2016年3月来我院就治的血液透析合并低血压患者60例作为研究对象。以专病一体化管理理念作为对患者的核心管理程序。比较管理前后患者的护理考核指标达标比例、患者的透析指标达标比例的变化、血液透析相关并发症发生率的情况。 结果 护理考核6各指标，在管理后患者饮食控制达标比例、遵医用药达标比例、门诊随访达标比例、微信使用达标比例、知识考核达标比例、满意度达标比例均与管理之前比较有明显的提高（P0.05）；患者的血压控制达标比例、血清蛋白达标比例、血红蛋白达标比例、血磷达标比例均比管理前明显升高（P0.05）；患者出现失衡综合征发生率、血量减少发生率、感染发生率和低血压发生率与管理前比较明显降低（P0.05）。 结论 采用专病一体化管理理念可提高血液透析合并低血压患者临床效果，改善患者血液透析指标，降低患者透析并发症的发生率，值得在临床上推广及应用。 　　[关键词] 专病；一体化；血液透析；低血压 　　[中图分类号] R277.5 [文献标识码] A [文章编号] 2095-0616（2017）05-81-04 　　[Abstract] Objective To explore the effect of integrated management concept of special diseases in hemodialysis patients with hypotension. Methods 60 cases of hemodialysis patients with hypotension in our hospital from March 2013 to March 2016 were selected as the research object.The concept of integrated management of special disease was the core management procedure of patients.The compliance ratio of nursing assessment indicators，the compliance ratio o dialysis index，and the complication rate of hemodialysis related were compared before and after the management. Results There were 6 indicators of nursing assessment.After management，the proportion of patients’ dietary control compliance，compliance with medication standards，the proportion of outpatient follow-up compliance，the proportion of compliance with the use of WeChat，the proportion of knowledge assessment standards，and satisfaction ratio had been significantly improved （P0.05）.The control rates of patients blood pressure，serum protein，hemoglobin，and serum phosphorus were significantly higher（P0.05）.The incidence of imbalance syndrome， the incidence of blood loss，the incidence of infection and the incidence of hypotension were significantly reduced （P0.05）. Conclusion The integrated management concept of special diseases can improve the clinical effect of hemodialysis patients with hypotension，improve the hemodialysis index，and reduce the incidence of dialysis complicatio
