人工真皮修复拇指指端缺损的临床分析 　　【摘要】 目的：探讨人工真皮修复拇指指端缺损的临床疗效。方法：选择2013年12月-2015年3月11例伴有骨或肌腱外露的拇指指端缺损患者作为研究对象，彻底清创后，采用人工真皮缝于创面，术后3周予以拆除人工真皮硅胶层。结果：11例拇指指端缺损伴骨或肌腱外露创面肉芽生长良好，创面自然上皮化。随访4～24个月，患指外形美观，指端两点分辨觉为4～6 mm。手功能优9例，良2例，优良率为100%。结论：人工真皮修复拇指指端缺损，操作简单，不牺牲供区，能最大限度地保存患指伤后的长度、功能及外观，是一种修复拇指指端缺损的理想方法。 　　【关键词】 人工真皮； 指端缺损； 拇指； 修复 　　Clinical Analysis of Artificial Dermis in the Repair of Thumb-tip Defect/LONG Hang，XU Jia-li，HUANG Yin-hao，et al.//Medical Innovation of China，2016，13（10）：047-050 　　【Abstract】 Objective：To investigate the clinical efficacy of artificial dermis in the repair of thumb-tip defect.Method：11 patients with bone or tendon exposed thumb-tip defect from December 2013 to March 2015 were selected as the research objects.After complete debridement，artificial dermis was sutured on the surface of wound.The artificial dermis silicon layer was removed in 3 weeks after operation.Result：In these 11 patients with bone or tendon exposed thumb-tip defect，granulation grew well，and the surface of wound developed epithelization naturally.After follow-up for 4 to 24 months，the wounded finger was found good-looking with the fingertip discrimination being 4 to 6 cm.The hand function of 9 cases were excellent，2 cases were fine，the good rate was 100%.Conclusion：It is easy and simple to repair thumb-tip defect with artificial dermis.This technique sacrifices no donor site and can maintain the length，the function and the appearance of wounded finger to the greatest extent，thus being an ideal method for the repair of thumb-tip defect. 　　【Key words】 Artificial dermis； Fingertip defect； Thumb； Repair 　　First-author’s address：The First People’s Hospital of Bijie City，Bijie 551700，China 　　doi：10.3969/j.issn.1674-4985.2016.10.014 　　手是人的第二张脸，手指是人的第二双眼睛，其中拇指功能占手部功能的40%～50%[1-2]。可见拇指的重要性。拇指指端缺损是手外科中常见的损伤，常伴有骨、肌腱外露，由于拇指指端缺损对手的功能和外观都产生重要的影响，必须重视其修复治疗，治疗应以最小的创伤修复方式为首选，以最大限度保留拇指伤后的长度、功能及外观为目的。2013年12月-2015年3月，笔者采用人工真皮修复拇指指端缺损11例，取得满意的疗效，现报道如下。 　　1 资料与方法 　　1.1 一般资料 选择2013年12月-2015年3月11例拇指指端缺损患
