家庭医生签约式服务对患者健康管理效果分析 　　摘 要 目的：研究家庭医生签约式服务在改善患者健康方面的效果，为今后开展社区健康管理服务提供相关依据。方法：2014年9月―2015年9月，随机抽取180名社区患者，对观察组90人提供家庭医生签约式服务，对照组90人提供常规社区健康服务，12个月后对比相关健康指标，并评价家庭医生签约式服务的干预效果。结果：经干预后，观察组与对照组在选择社区门诊就医的比例及认为社区就医费用便宜，差异具有统计学意义（P0.05）。观察组与对照组在掌握慢性病常识、愿意改变原有生活行为方式、合理饮食、适量运动、戒烟戒酒、自我监测及情绪调节各方面相比差异均有统计学意义（P0.05）。结论：采用家庭医生签约式服务，能增强患者的自我健康保健意识，增加对健康管理的依从性，值得推广应用。 　　关键词 家庭医生 签约式服务 健康管理 　　中图分类号：R197.1 文献标志码：A 文章编号：1006-1533（2016）14-0052-03 　　Analysis of the effect of the family doctor contract service on the health management of patients 　　PENG Lei， ZENG Guoqing 　　（ Xujiahui Community Health Service Center of Xiuhui District， Shanghai 200030， China） 　　ABSTRACT Objective： To study the effect of the family doctor contract service in improving the health of patients to provide the basis for the development of community health management services in the future.
