脓毒症患者血清降钙素原及肌钙蛋白I水平检测临床意义 　　[摘要] 目的 检测脓毒症患者血清降?}素原（procalcitonin，PCT）和肌钙蛋白I（Cardiac troponin I，cTnI）的浓度水平，探讨其在脓毒症中的临床意义。 方法 选择2015年11月～2016年10月就诊于本院的108例脓毒症患者作为研究对象，按病情轻重将其分为脓毒症组（n=38）、严重脓毒症组（n=41）、脓毒性休克组（n=29），并按临床结局分为存活组（n=91）和死亡组（n=17），分析和比较不同组别之间血清PCT 和cTnI浓度水平的变化。 结果 脓毒症组患者的PCT和cTnI分别为（1.77±0.42）ng/mL 和（0.08±0.02）ng/mL，严重脓毒症组分别为（4.09±1.70）ng/mL和（0.28±0.09）ng/mL，脓毒性休克组分别为（6.82±2.23）ng/mL和（0.63±0.22）ng/mL。脓毒症病情越严重，PCT和cTnI浓度水平越高，差异有统计学意义（P0.01）。死亡组的PCT 和cTnI分别为（6.29±1.82）ng/mL和（0.75±0.14）ng/mL，存活组分别为（3.58±0.73）ng/mL和??0.22±0.08）ng/mL，死亡组的PCT及cTnI水平明显高于存活组（P0.01）。 结论 血清PCT及CTnI浓度水平测定对脓毒症患者病情严重程度与预后判断具有指导意义。 　　[关键词] 降钙素；肌钙蛋白I；脓毒症；病情严重程度 　　[中图分类号] R51；R63 [文献标识码] B [文章编号] 1673-9701（2017）15-0088-03 　　[Abstract] Objective To investigate the concentration level of procalcitonin（PCT） and cardiac troponin I（cTnI） in sepsis patients， and to explore its clinical significance in sepsis. Methods A total of 108 patients with sepsis admitted in our hospital from November 2015 to October 2016 were enrolled in this study. They were divided into sepsis group（n=38）， severe sepsis group（n=91） and septic shock group（n=29）. And they were also divided into survival group（n=91） and death group（n=17） according to clinical outcome.The levels of serum PCT and cTnI in different groups were analyzed and compared. Results The levels of PCT and cTnI in the sepsis group were （1.77±0.42） ng/mL and （0.08±0.02） ng/mL respectively， and those of the severe sepsis group were （4.09±1.70） ng/mL and （0.28±0.09）ng/mL， with（6.82±2.23） ng/mL and （0.63±0.22） ng/mL in the septic shock group， respectively. The more severe sepsis， the higher the level of PCT and cTnI concentrations， and the difference was statistically significant（P0.01）. The levels of PCT and cTnI in the death group were （6.29±1.82） ng/mL and （0.75±0.14） ng/mL respectively， and those of the survival group were （3.58±0.73） ng/mL and （0.22±0.08） ng/mL. The level of PCT and cTnI was significantly higher than that of survival group（P0.01）.
