腹腔镜手术治疗化脓穿孔性阑尾炎合并腹膜炎临床观察 　　[摘要] 目的 研究腹腔镜手术治疗化脓穿孔性阑尾炎合并腹膜炎的临床效果。方法 选取本院2010年3月~2013年7月收治的58例化脓穿孔性阑尾炎合并腹膜炎患者，随机将其分为两组，每组各29例。观察组行腹腔镜手术，对照组行开腹手术，比较两组的相关指标。 结果 两组的手术时间差异无统计学意义（P0.05）；观察组的总有效率高于对照组，术后排气时间、住院时间、下床活动时间均短于对照组，止痛药物使用率、残余脓肿率、切口感染率、置引流管率均低于对照组（P0.05）。 结论 腹腔镜手术治疗化脓穿孔性阑尾炎合并腹膜炎，效果显著，可缩短住院时间，减轻患者的疼痛，值得推广应用。 　　[关键词] 腹腔镜手术；化脓穿孔性阑尾炎；腹膜炎 　　[中图分类号] R656.8[文献标识码] A[文章编号] 1674-4721（2014）06（c）-0019-03 　　 　　Clinical observation of laparoscopic surgery in the treatment of suppurative and perforated appendicitis complicated with peritonitis 　　CHEN Zhen CHEN Yong-qiang YE Bi-zhen 　　Department of General Surgery,People′s Hospital of Fengshun County in Meizhou City of Guangdong Province,Fengshun 514300,China 　　[Abstract] Objective To study the clinical effect of laparoscopic surgery in the treatment of suppurative and perforated appendicitis complicated with peritonitis. Methods 58 patients with suppurative and perforated appendicitis complicated with peritonitis admitted into our hospital from March 2010 to July 2013 were selected and they were randomly divided into two groups,and there were 29 cases in each group.The observation group was given laparoscopic surgery,while the control group was given laparotomy.The related indexes of the two groups were compared. Results There was no statistical difference of operation time in the two groups (P0.05).The total effective rate in the observation group was higher than that in the control group,the postoperative time of exhaust,hospital stay,time of getting out of bed in the observation group were shorter than those in the control group,analgesic drug use rate,rate of residual abscess,incision infection rate, drainage tube rate in the observation group were lower than those in the control group (P0.05). Conclusion The effect of laparoscopic surgery treating suppurative and perforated appendicitis complicated with peritonitis is significant,and it can shorten hospital stay and alleviate patients′ suffering.It is worthy of pr
