基层医院开展降钙素原肺炎支原体抗体检验的项目对呼吸道感染病人平均住院日及费用统计分析 　　[摘要] 目的 探讨基层医院开展降钙素原（PCT）及肺炎支原体（MP）的检测对小儿呼吸道感染住院儿童的住院日及住院费用的影响。方法 整群选取晋江市医院晋南分院2015年1―9月所有诊断呼吸道感染性疾病的儿科住院病历共3 026份，分为未检测PCT、MP组（1 261例）、同时检测PCT、MP组（765例）、检测PCT组（169例）、检测MP组（831例），进行回顾性统计分析，了解各组间的平均住院日、次均费用是否存在差异。结果 平均住院日方面：未检测PCT、MP组（5.16 d）与其他3组差异有统计学意义（P0.05）。未检测PCT、MP组与同时检测PCT、MP组（2 058.78元）、检测MP组（1 545.75元）比较差异有统计学意义（P0.05）。结论 基层医院开展PCT或MP的检测，有助于减少病人住院日，避免滥用抗生素，减轻住院费用或药费，加强基层医院合理用药管理，提升基层医生合理使用抗生素的依从性。 　　[关键词] 降钙素原；肺炎支原体；呼吸道感染；住院日；住院费用 　　[中图分类号] R446 [文献标识码] A [文章编号] 1674-0742（2016）08（a）-0077-04 　　[Abstract] Objective To analysis the influence of detection of calcitonin and/or mycoplasma pneumonia antibody on expenses and days of hospital stay of the acute respiratory infection pediatric patients in primary care setting. Methods Group selection retrospective analysis expenses and days of hospital stay among acute respiratory infection pediatric patients hospitalized in Jinnan Branch of jinjiang hospital from January to September 2015，all 3 026 clinical cases enrolled. All enroll patients were divided into four groups （Calcitonin and Mycoplasma Pneumonia Antibody undetected 1 261 cases， combined Calcitonin and Mycoplasma Pneumonia Antibody detected 765 cases， Calcitonin detected 169 cases and Mycoplasma Pneumonia Antibody detected 831 cases. Results The average length of stay in Calcitonin and Mycoplasma Pneumonia Antibody undetected （5.16 days）has a significant difference compared with other three groups. Medical expenses in Calcitonin and Mycoplasma Pneumonia Antibody undetected group（1 853.04 yuan）has a significant difference compared with combined detected group （2 058.78yuan） and Mycoplasma Pneumonia Antibody detected group（1 545.75yuan）. Conclusion Detection of Calcitonin and / or Mycoplasma Pneumonia Antibody could short average length of hospital stay and decrease medical expense and guide antibiotics use in primary care setting. 　　[Key words] Calcitonin ； Mycoplasma； Acute Respiratory ； Such confinement
