COPD肺康复患者疲乏相关性因素分析 肖新丽(湖南永州职业技术学院425006) 】R563 】A 】1672-5085 (2011) 40-0085-03 】目的观察COPD肺康复患者疲乏相关因素，并研究其与呼吸网难、睡 眠质量和生理方面的关系。方法选择48名COPD肺康复患者，应用以下几种 问卷调查方法：疲乏因素应用多维疲乏量表(MFI);焦虑和抑郁应用医院焦虑抑 郁量表(HADS),睡眠质量应用匹兹堡睡眠质量指数(PSQI)。结果95.8%肺康复 病人有体力疲乏，精神疲乏和一般性疲乏分别为72.9%和50.0%。41.7%的患者有 焦虑表现，20.8%有抑郁表现。结论疲乏尤其是体力疲乏在大部分COPD肺康 复病人中可见，并影响肺康复措施的各个方面。 关键词】COPD疲乏焦虑和抑郁睡眠质量相关性 The Analysis of Fatigue Related Factors in COPD Patients Abstract】 Objective:To examine dimensions of fatigue such as the emotional, behavioral, cognitive, and physical, and their relationships to dyspnea, mental health, sleep, and physiologic factors.methods: A convenience sample of 48 pulmonary rehabilitation patients with COPD completed self-report questionnaires, including: dimensions of fatigue using the Multidimensional Fatigue Inventory(MFI), anxiety and depression using the Hospital Anxiety and Depression Scale (HADS) , and sleep quality using the Pittsburgh Sleep Quality Index(PSQI). Data on otherclinical variables were abstracted from pulmonary rehabilitation program health records.Results: Almost all (95.8%) patients experienced high levels of physical fatigue. 87.5%of fatigue were also reported for the dimensions of reduced activity, reduced motivation (83.3%), mental fatigue (72.9%), and general fatigue (50.0%). 41.7% patients reported symptoms of anxiety, while 20.8% patients reported depressivesymptoms. Age was related to the fatigue dimensions of reduced activity (r=0.41, Plt;0.05) andreduced motivation(r=0.34, Plt;0.05). Anxiety was related to reduced motivation(r=-0.45, Plt;O.Ol). Fatigue was not associated with symptoms of depression, sleep quality, gender,supplemental oxygen use, smoking status, or Medical Research Council dyspnea scores.Conclusions: Fatigue particularly the physical and reduced motivation dimensions of fatigue，was experienced by almost all COPD patients attending this pulmonary rehabilitationprogram.
