脉搏波传导速度、动脉顺应性在2型糖尿病肾病中的变化及影响因素 ［ ］H的探讨2型糖尿病肾病患者PWV （脉搏波传导速度）、C1 （大动脉顺应性指数）的变化及其影响因素。方法选取2016年1一 10月于重庆医科大学附属第二医院肾内科住院的2型糖尿病肾病患者 180例为病例组，将病例组依据24 h尿蛋0及eGFR水平分为早期糖 尿病肾病组、临床糖尿病肾病组、终末期糖尿病肾病3个亚组。选取 同期于该院体检的健康成年人144名为对照组。测定所存患者血清白 蛋白、血尿素氮、血肌酐、甘油三酯、总胆固醇、高密度脂蛋白、低 密度脂蛋白、糖化血红蛋白、24 h尿蛋白、平均动脉压、PWV、C1。结 果与对照组相比，病例组PWV升高，且终末期糖尿病肾病组高于临床 糖尿病肾病组及早期糖尿病肾病组；病例组C1较对照组降低，且终末 期糖尿病肾病组低于临床糖尿病肾病组及早期糖尿病肾病组。PWV与 血清白蛋白、血肌酐、甘油三酯、高密度脂蛋白、尿蛋白、口间及夜 间平均动脉压相关；C1与血清白蛋白、血肌酐、总胆固醇、尿蛋白、 円间及夜间平均动脉压相关。结论2型糖尿病肾病患者随着疾病发 展，动脉硬化程度逐渐增加，多种危险因素影响动脉硬化的发生 ，供学习和研宄 使用， 和著作权归原 所有，如有不 愿意被转载的情况， 己 ［关键词］糖尿病肾病；动脉硬化；脉搏波传导速度；大动脉顺应性 ［ ］R692 ［ ］A ［ ］1672-4062 (2016) 12 (b) -0007-04 ［Abstract］ Objective To investigate the change and related factors of PWV (pulse wave velocity), Cl (large arterial compliance index) in the patients with diabetic nephropathy. Methods Select January 2016 to October 2016, According to urinary albumin excretion and the level of estimated glomerular filtration rate (eGFR )，180 patients with diabetic nephropathy were divided into early diabetic nephropathy group， clinical diabetic nephropathy group and end stage renal disease group ， and 144 healthy people were enrolled as control group. Serum albumin, serum creatinine (Scr), blood urea nitrogen, blood lipids, urinary protein, 24-hour ambulatory blood pressure, PWV and Cl were determined. Results Compared with the control group, cases increased PWV and end-stage diabetic nephropathy group is higher than clinical diabetic nephropathy and early diabetic nephropathy group； Case group Cl reduced than the control group, and clinical diabetic nephropathy end-stage diabetic nephropathy group is lower than group and early triglycerides, high-density lipoprotein cholesterol (hdl-c)， urinary albumin, mean arterial pressure during the day and night ； Cl and serum albumin， serum creatinine， total cholesterol, urine protein, daytime and nighttime is related to the mean arterial pressure.
