Laparoscopic radical resection has been accepted as one of the standard treatment protocol for stage IA in the latest version of Japanese gastric cancer treatment guidelines. The success of laparoscopic radical resection in early gastric cancer promoted the use of laparoscopic D2 radical resection in advanced gastric cancer by some scholars. In 1997, Goh carried out the first laparoscopic D2 radical gastrectomy for gastric cancer. However, laparoscopic D2 radical gastrectomy for advanced gastric cancer developed less satisfied, due to the complicated stomach anatomy and lymph node metastasis pathway, in along with the difficulty of the digestive tract reconstruction. Meanwhile, the other reason was whether laparoscopic operation would promote the invasion of gastric cancer was still in debate. The laparoscopic radical gastectomy for gastric cancer, particularly for advanced gastric cancer, was initially carried out in our center in 2004. Amount of creative and effective techniques were created by our center. The laparoscopic operation approach, lymph node dissection technique, and reconstruction of digestive tract were partially handled. According to the data of retrospective studies from our center and others, the effect of the laparoscopic radical gastectomy in early stage gastric cancer was confirmed (including the safety, effectiveness, and recent therapeutic effect).
