彩色多普勒超声在动静脉内痿及其并发症中的应用价值 王瑜 （攀枝花市中西医结合医院特检科 四川 攀枝花617000） 【摘 要】目的：探讨彩色多普勒超声在透析患者自体动静脉内痿并发症中 的应用价值。方法：采集2012年10月至2015年5月应用彩色多普勒超声检测 46例透析患者楼动脉■头静脉内痿痿口及挠动脉、头静脉的二维超声表现及血流 动力学状况。结果：46例透析患者动静脉内痿中，28例动静脉内痿通畅；18例 出现并发症：□例血栓形成（其中并狭窄9例，并静脉闭塞2例，并静脉瘤样 扩张2例，并周围血肿压迫1例，并肿胀手综合征1例）；1例单纯头静脉旁血 肿压迫致狭窄；1例痿口狭窄；1例头静脉多发性瘤样扩张并血流量下降；1例 内痿发育差，头静脉血流量不足；1例假性动脉瘤；2例内痿闭合。结论：彩色 多普勒能够早期明确诊断动静脉内痿并发症，并快速诊断头静例脉低血流量原因, 从而为临床及时治疗提供及时准确的依据。 【关键词】彩色多普勒超声；动静脉内痿；并发症 【中图分类号1R543.6 【文献标识码】B 【文章编号11003-5028（2015 ） 8?0542-01 Application value of Color Doppler ultrasound in arteriovenous fistula and its complicatio ns [Abstract] Objective To investigate the application value of color Doppler ultrasound in the complications of autologous arteriovenous fistula in hemodialysis patients. Methods 46 cases of radial artery fistula in hemodialysis patients were detected and the two-dimensional ultrasound and hemodynamic status of radial artery and head vein were collected by color Doppler ultrasound from October 2012 to May 2015. Results in 46 cases of dialysis patie nts with in ter nal arteriove nous fistula, 28 cases of dynamic venous fistula patency, complications occurred in 18 cases: 11 cases of thrombus formed (and stenosis in 9 cases, and venous occlusion in 2 cases and venous aneurysm expansion in 2 cases, and around hematoma in 1 case, and swelling of the hand syndrome (1 case), 1 cases of pure single head vein near the hematoma compressio n caused by ste no sis, 1 case fistula steno sis, 1 cases of cephalic vein multiple tumor like dilatation and blood flow decreased, 1 case fistula development is poor, insufficient blood flow to the cephalic vein, one case of pseudoa neurysm, in 2 cases and in ter nal fistula. Con clusi on Color Doppler can clearly diagnose the complications of arteriovenous fistula in the early stage, and rapid diagnosis of the cause of low blood flow in the radial head vein, so as to provide timely and accurate basis for clinical treatment. 【Keywords］ Color Doppler ultrasound* arterioveno
