Patients with Heart Failure (HF) have a reduced exercise capacity. Taking into account that beta-blockers are prescribed in low doses and often can not be uptitrated due to tolerability reason (fatigue, low BP), the authors wanted to know if 伊伐布雷定 could offer opportunity to improve the exercise capacity and the quality of life in these patients. * This study is a randomized, open, blinded endpoint study performed to assess the effect of Heart Rate reduction with Carvedilol up to 25 mg bid (n=38), 伊伐布雷定 up to 7,5 mg (n = 41), and their combination 12,5/7,5 mg bid (n = 42), on exercise capacity in Heart Failure patients receiving maximal dose of ACE inhibitor. Inclusion criteria: 18 Years and older heart failure II-III ischemic and non ischemic origin stable medications from at least 3 months discharged in stable conditions 3 months from an acute ischemic syndrome or revascularisation procedure naive on heart rate reducing agents or on suboptimal beta-blocker/ACE-inhibitor dose 6MWT tolerance 100 600 m Reproducible 6MWT * Results showed that 伊伐布雷定 alone or in combination with carvedilol is more effective than carvedilol alone at improving exercise tolerance in HF patients: Heart rate was reduced in all three groups, but to a greater extent by the combination. The distance walked on the 6-min walking test and the exercise time on MVO2 test significantly improved in the 伊伐布雷定 and combination groups, as did peak VO2 and VAT. No changes in these parameters were found with carvedilol.
