中流量吸氧联合抗生素应用在复发性自发性气胸治疗中作用的临床观察 熊胜春 ［摘 要］目的：探讨屮流量吸氧联合抗生素应用治疗复发的白发性气胸 患者的临床疗效；方法：采取回顾性分析方法纳入自发性气胸经胸腔闭式引流治 疗结束后7天内复发的患者40例；治疗组20例；对照组：20例；治疗组接受 胸腔闭式引流术后给予应用抗生素静点4天，拔除胸腔引流管前后给予持续吸氧; 对照组接受胸腔闭式引流术后预防性应用抗生素静点治疗24小时，常规拔除胸 腔闭式引流管；结果：治疗组二次引流术后拔管时间明显短于对照组，拔管后复 发率明显低于对照组,两组对比，差异有统计学意义(plt;0.05)?结论：低流量 吸氧结合抗生素应用对复发性气胸的治疗有显著效果，值得临床尝试和推广。 ［关键词］自发性气胸 抗生素 屮流量吸氧 D0l:10.3760/cma.j.issn.0376-2491.2014.44.050 作者单位：067000河北 承德，承德市中心医院心胸外科 The Clin ical Effect of Medium Flow Oxyge n In halation Associated with Antibiotics Application in the Treatment of Recurrent Spontaneous Pneumothorax Xiong she ng chun Abstract: Objective To investigate the clinical effect of medium flow oxygen inhalation associated with antibiotic therapy in patients with recurrent spontaneous pneumothorax? Method The data of 40 spontaneous pneumothorax patients who had recurrence after tube drainage within 7 days were retrospectively analyzed and divided into the experimental and control group, each with 20 cases. The pleural drainage duration and the recurrence rate between the two groups were compared. The experimental group was treated with tube drainage and intravenous antibiotic preventatively in the following four days; the control group was treated with tube drainage too, but only applied intravenous antibiotic therapy for 24 hours. The patients in experimental group used nasal catheter oxygen inhalation with medium flow peri-extracting drainage tube, while the control group extracted the drainage tube con venti on ally. Result The extubation time in the experimental group was significantly shorter than the control group after the second tube drainage, and the recurrenee rate in the experimental group was obviously lower than the control group, both of which were sign ifica ntly d iff ere nt,(Plt;0.05). Conclusion Medium flow oxyge n in halati on associated with antibiotics has significant curative effect on the recurrent spontaneous pneumothorax, which is worthy clinical promotion.
