心电监护引导下PICC末端定位的临床研究 张蕾郭素萍(通讯作者)南通大学附属泰州市人民医院肿瘤科江苏泰州 225300 【摘 要】目的：探讨床边心电监护引导下置入PICC导管头端到位率。方法： 将木科室2014年1月至8月己行PICC置管，胸片定位的患者51例为对照组； 2014年10月至2015年3月行床边心电监护引导PICC置管51例为实验组, 运用接有心电监护仪的导电夹来连接导丝的方法，通过监护仪P波振幅的改变 来确定导管在上腔静脉内的最佳位置，并通过X线摄片证实。 比较两组患者导管尖端的到位率及可调导管花费的调管时间。结果：实验组 导管尖端到位率为94%,对照组导管尖端到位率为74.5%,两组比较差异有统计 学意义。(PV0.05)；实验组可调导管的时间为18plusmn;2.16min,对照组可调 导管的时间为90plusmn;12.48min,两组时间比较有统计学差异。(P0.05)结 论：床边心电监护引导下置入PICC导管，能显著提高PICC导管尖端的到位率， 避免经验法体表预测导管长度的的误差，操作简便、准确性高，有望成为临床置 管定位的规范。 【关键词】EKG； PICC置管；导管头端定位[abstract] Objective： to study the bedside ecg monitoring must be placing PICC catheter guided head end.Methods： the undergraduatecourse room in January 2014 to August have line of PICC catheter, 51 patie nts with chest radiograph positi oning as the control group； In October 2014 to March 2015 lines of bedside ecg monitoring guide 51 cases of PICC catheter for the experimental group, using the clip toeonnect the electric after a heart monitor method of thread through the monitor P wave amplitude changes to determine the catheter is in the best positio n inside the superior vena cava, which is verified by X-ray radiography.Comparison of two groups of patients with cathetertip must be and adjustable catheter tube time cost.Results： the catheter tip must be 94%, control group catheter tip must be 74.5%, is similarbetween the two groups have statistical sign讦icance.
