TEAMING AGREEMENT Enter the month, day, year and Team Member name in the following spaces below. THIS AGREEMENT (this “Agreement”) is made and entered into as of ?, 19 ?, by and between ARTHUR ANDERSEN LLP (hereinafter referred to as AA) and ? (hereinafter referred to as the Team Member).
