2014-1-23主任加工版 胸腔镜手术与剖胸手术治疗自发性气胸比较的系统评价 吴正国1，蔡庆勇2 （1.深圳市宝安区松岗人民医院，广东 深圳 518000； 2.遵义医学院，贵州 遵义 563000） [摘要]目的：系统评价电视胸腔镜手术（VATS）与标准剖胸手术(CTH)治疗自发性气胸的临床疗效。方法：运用Meta分析方法，综合定量分析国内公开发表的有关自发性气胸患者采用电视胸腔镜手术与标准剖胸手术治疗的10篇文献资料。结果：Meta分析结果显示：①手术时间VATS组少于CTH组（P＜0.01）；②住院时间VATS组明显短于CTH组（P＜0.01）；③术中出血VATS组明显少于CTH组（P＜0.01）；④拔除胸管时间VATS组优于CTH组（P＜0.01）；⑤并发症发生率VATS组明显低于CTH组（P＜0.01）；⑥术后复发率VATS组低于CTH组（P＜0.0≥）。结论：电视胸腔镜手术治疗自发性气胸比标准剖胸手术治疗有明显优势，缩短手术时间和住院时间，减少术中出血，降低并发症的发生，减少术后复发。 [关键词]电视胸腔镜手术；标准剖胸手术；自发性气胸；Meta分析 [中图分类号]R [文献标志码]A doi:10.11851/j.issn.1673-1557.2014.01.0 A Comparison of Video-Assisted Thoracoscopic Surgery with Conventional Thoracotomy in Treatment of Spontaneous Pneumothorax：A Systematic Review Wu Zhengguo， (1.Shenzhen Baoan Songgang People抯 pHospital, Guangdong Shenzhen 518000,China; 2.Zunyi Medical College,Guizhou Zunyi 563000,China) [Abstract]Objective:To evaluate the clinical effectiveness of Video-assisted thoracotomy and conventional thoracotomy in spontaneous pneumothorax.Methods:By meta-analysis we made a comprehensive analysis of 10 literatures published in China about Video-assisted thoracotomy and conventional thoracotomy in in spontaneous pneumothorax.Results:The results of meta-analyses showed that:①There were significant differences in the operative time（P＜0.01）,②in the hospitalization time（P＜0.01）,③in the bleeding loss（P＜0.01）,④in the drainage tube removal time（P＜0.01）,⑤in the complications（P＜0.01）,⑥in the recurrence rate between the two groups（P＜0.05）.Conclusions:Video-assisted thoracotomy are superior to the conventional thoracotomy in operative time,length of hospital stay,bleeding loss and decrease in complications and recurrence rate.
