食管癌高危人群食管上皮炎症细胞浸润与增生关系的初步病理学研究_医学论文 食管癌高危人群食管上皮炎症细胞浸润与增生关系的初步病理学研究_医学论文 【摘要】 目的：初步分析食管癌高危人群内镜抽样普查食管黏膜上皮病变特点并探讨食管黏膜上皮内浸润的炎症细胞数目与上皮增生的关系。方法：对林州市合涧乡1492名无明显症状的自愿者行内镜检查并对可疑病变组织多点取材。对其中的1480例食管黏膜上皮病理诊断进行初步统计分析,并随机抽取262例食管黏膜上皮组织，高倍镜下观察上皮内的炎症细胞浸润数目与上皮增生程度之间的关系。结果：非典型增生的检出率在40～49岁年龄组为18％(120/638)、50～59岁年龄组为31％(208/670)、60～69岁年龄组为33％(57/172)；食管癌的检出率在各年龄组分别为0、0％(4/670)和1％(3/172)。在病理诊断的正常上皮、单纯增生、轻度非典型增生、中度非典型增生、重度非典型增生的上皮中，上皮内浸润的炎症细胞每高倍视野下大于10的检出率分别为：7％(9/120))、38％(29/75)、43％(17/39)、81％(17/21)、85％(6/7)。结论：食管上皮单纯增生以及非典型增生与上皮内浸润的炎症细胞数目具有较为明显的相关性。 【关键词】 上皮内炎症细胞浸润 上皮增生 食管炎症 ［Abstract］Objective: To analyze the pathologic changes of mucous membrane of esophagus from high_risk population of esophageal carcinoma by endoscopic screening, and to evaluate the connection of the numbers of intraepithelial infiltrating inflammatory cells and epithelial proliferation. Methods：The samples from 1 492 symptom_free subjects living in Hejian town, Linzhou citywere collected, and the pathologic diagnosis was made in 1 480 samples and analyzed initially. In 262 subjects selected randomly, the numbers of the inflammatory cells in the mucous membrane were counted under high power lens, and then the association between the numbers of inflammatory cells and the stage of epithelial proliferation was analyzed. Results: The prevalence of atypical hyperplasia was 188％(120/638)in the subjects from 40 to 49 years, 31％(208/670)from 50～59 years and 33％(57/172)from 60～70 years, respectively and the prevalence of esophagus cancer was 0％(4/670)and 1％(3/172)in the different age stages mentioned above. In the groups of normal、hyperplasia、mild atypical hyperplasia、moderate atypical hyperplasia and severe atypical hyperplasia, the incidence of samples with inflammatory cells over 10 per high power field was 75％(9/120), 38％(29/75), 43％(17/39), 81％(17/21)and 85％(6/7), respectively.
