  带血运的筋膜瓣包绕同种异体骨修复大段骨缺损的生物力学 赵晓东 范启申 高学建 赵玉祥 （解放军第 89 医院创伤骨科研究所，山东 潍坊 261021 ） 摘要 【目的】 通过生物力学测试和分析，探讨应用带血运的筋膜瓣包绕深低温冷冻处理的 同种异体骨修复大段骨缺损的效果。【方法】取健康成年新西兰大白兔20只，随机分2组，每组 10 只，实验组筋膜瓣包绕同种异体骨段，对照组单纯植入冷冻处理的同种异体骨。术后 12 周处 死取材行三点弯曲试验和扭转试验检测各组移植骨的生物力学特性。【结果】 兔骨移植术后12w 实验组最大弯曲力、最大抗压强度均高于对照组(P0.05)；最大扭矩、抗扭强度实验组均高于对 照组(P0.05)。 【结论】带血运的筋膜瓣包绕同种异体骨修复大段骨缺损，其生物力学效能明显 优于对照组。 关键词 骨缺损；同种异体骨；筋膜瓣；移植；生物力学 Abstract 【Objective 】To evaluate the repair ability of bone defect that treated with allograft combined with fascial flap through the test of vitodynamics test that was observed in the repair process, so we could evaluation of effort。【Methods】 Twenty healthy rabbits were choosen. We divided them in two groups in random and each group had 10 rabbits. In experimental group we transplanted allogenic bone, frist we cut abdominal shallow vascular pedicled fascial flap, then used it to wrap the middle femur bone and implanted; After 12 weeks, examined by the Three point bending test and theTorsion test. SPSS16.0 statistical software were used for analysis of test data. 【Results】Three point bending test showed that: maximum bending strength, maximum compressive strength of the experimental group were higher than group(P0.05)；Torsion test showed that : maximum torque, torsional strength of the experimental group were higher than group(P0.05). 【Conclusion】 Allogenic bone combined with fascial flap repair the bone defect better. Keywords bone defect; allogenic bone; fascial flap; transplantation; biomechanics detection 大段骨缺损的修复一直是骨科普遍而棘手的问题，尤其是长段负重骨缺损，其重建不仅需要移 植物具有足够的长度，而且需要有足够生物力学性能，因而其修复变得更加困难。自体骨移植难以 同时获得足够的长度和力度，供骨在外形上也难于与受区相一致。同种异体骨具有来源丰富，并具 有一定的生物学活性和生物相容性，移植时限灵活，经处理的同种异体骨移植后免疫排斥反应明显 减弱，能提供较好的结构支持，具有良好的骨传导作用，因此具有一定的临床应用前景。但异体骨 移植后如何加强生物力学强度成为当前临床上急需解决的问题。本实验通过筋膜瓣包绕异体骨修复 骨缺损观察骨修复过程以及生物力学测试，评价应用的效果，为临床大段负重骨缺损的修复提供依 据。 1.材料与方法 1.1实验动物及分组 健康成年新西兰大白兔20只，平均月龄6个月，雌雄不限，体重2.0～3.0kg，
