腔内彩色多普勒超声对剖宫产切口瘢痕妊娠的临床价值 　　【摘要】 目的 探讨腔内彩色多普勒超声在剖宫产切口瘢痕妊娠诊断中的临床应用价值。方法 选取于2011年6月～2013年6月入本院治疗的45例剖宫产切口瘢痕妊娠患者为研究对象， 对上述患者分别行经阴道腔内彩色多普勒超声检查和腹部彩色多普勒超声检查， 比较两种检查方法的显像率， 观察检查显像结果。结果 阴道腔内彩超显像44例， 其中包括25例妊娠囊型， 16例包块型和3例胚胎型， 1例漏诊， 显像率为97.78%；腹部彩超检查显像32例， 显像率为71.11%。两组检查方法的超声显像率比较差异具有统计学意义（P0.05）。超声检查结果显示， 宫腔内无妊娠组织38例， 瘢痕处肌层回声团比较明显， 呈现出杂乱、强弱不均等特点， 同时多处可见不规则液性回声， 子宫整体呈纺锤状。结论 经阴道腔内彩色多普勒超声检查为仔细观察病灶与周边血流关系提供了便利， 是剖宫产切口瘢痕妊娠的首选检查方法， 具有重要的临床应用价值。 　　【关键词】 彩色多普勒超声；剖宫产切口瘢痕妊娠；经阴道腔；经腹 　　【Abstract】 Objective To investigate the clinical application value of lumen of color doppler ultrasound in the diagnosis of cesarean section incision scar pregnancy. Methods choose in June 2011 to June 2013， works in our treatment of 45 cases of cesarean section incision scar pregnancy patients as the research object， through the vaginal cavity respectively in patients with the color doppler ultrasound and color doppler ultrasound belly， checking method of comparing two imaging rate， observe imaging inspection results. Results 44 cases of Vaginal cavity colour to exceed imaging，including 25 cases of gestational sac type，16 cases of bag piece type and 3 cases of embryonic type， 1 case in diagnosis imaging rate was 97.78%； 32 cases of abdominal colour to exceed imaging，imaging rate was 71.11%. Two sets of inspection method of ultrasonic imaging rate have obvious differences， statistically significant （P0.05）. Ultrasound examination， according to the results of intrauterine pregnancy group 38 cases， muscular layer echo mass obvious scar，present a mixed and disorderly， unequal strength characteristics， at the same time， several irregular fluid echo，uterine spindle on the whole.
