Arterialhypertension(HT)isagedependentand,withtheprolongationoflifeexpectancy,a?ectsmoreandmoreelderlypeople. Intheelderly,HTisariskfactorfororgandamageandcardiovascular(CV)events.Bothpharmacologicandnonpharmacologic reductionofbloodpressure(BP)isassociatedwithacorrespondingdecreaseinsystolic-diastolicorisolatedsystolicHT.Clinical trialshaveshownthatBPloweringisassociatedwithadecreaseinstrokeandotherCVevents.Therefore,BPreductionperse appearsmoreimportantthanaparticularclassofantihypertensivedrugs.Thebene?tofantihypertensivetreatmenthasbeen con?rmeduptotheageof80years,remainingunclearintheoctogenarians.Thebene?tinloweringdiastolicBPbetween80and 90mmHgiswellestablished,whilethatofloweringsystolicBPbelow140mmHgrequiresfurthercon?rmations.
