Peripheral microvascular impairment in systemic sclerosis (SSc) may be easily detected and scored in a safe noninvasive way bynailfoldvideocapillaroscopy(NVC).ThepaperhighlightsclinicalconditionsrelatedtoSScinwhichNVCmayrepresentan outcome measure of therapeutical interventions, by elaborating on their already assessed relationship with the NVC patterns andeventuallyscores.The3importantbiological/clinicalconditionsare:thepositivityforSSc-speci?cserumautoantibodies, thepresenceofSScskindigitalulcers(DUs)andofpulmonaryarterialhypertension(PAH)SScassociated.Inconclusion,tothe questionifcapillaroscopy(NVC)mayrepresentinSScanoutcomemeasureforclinicaltrialsontheperipheralvasculopathy,based onthegrowingevidenceandourdetailedstudies,theanswerispositive.RecenttherapeutictrialsinSScarecon?rmingthisrole, andtheexperienceisgrowingrapidly. 1.Introduction havebeenclassi?edin3validatedpatterns(early,active,and late)ofmicroangiopathybyNVCandscored(Figure2)[5– 7].
