Methods:TemperaturedatabetweenApril1995andJune2008werecollectedfrom588patientsandanalyzedin aretrospectivecohortstudybyobserversblindedtooutcome.Thetimeneededtoachieveanesophageal temperatureoflessthan34°Cwasrecorded.Survivalandneurologicaloutcomesweredeterminedwithinsix monthsaftercardiacarrest. Results:Themediantimefromrestorationofspontaneouscirculationtoreachingatemperatureoflessthan34°C was209minutes(interquartilerange[IQR]:130-302)inpatientswithfavorableneurologicaloutcomescomparedto 158min(IQR:101-230)(P0.01)inpatientswithunfavorableneurologicaloutcomes.Theadjustedoddsratiofora favorableneurologicaloutcomewithalongertimetotargettemperaturewas1.86(95%CI1.03to3.38,P=0.04). Conclusions:Incomatosecardiacarrestpatientstreatedwiththerapeutichypothermiaafterreturnofspontaneous circulation,afasterdeclineinbodytemperaturetothe34°Ctargetappearstopredictanunfavorableneurologic outcome.
