Recenttrialshavedemonstratedthatmoderatehypothermia,startedwithin6hoursafterbirthandprotractedfor 72hours,cansignificantlyimprovesurvivalandreduceneurologicimpairmentinneonateswithhypoxic-ischemic encephalopathy.Itisnotcurrentlyknownwhetherneuroprotectivedrugscanfurtherimprovethebeneficialeffects ofhypothermia.Topiramatehasbeenproventoreducebraininjuryinanimalmodelsofneonatalhypoxicischemic encephalopathy.However,theassociationofmildhypothermiaandtopiramatetreatmenthasneverbeenstudied inhumannewborns.Theobjectiveofthisresearchprojectistoevaluate,throughamulticenterrandomized controlledtrial,whethertheefficacyofmoderatehypothermiacanbeincreasedbyconcomitanttopiramate treatment. Methods/Design:Termnewborns(gestationalage≥36weeksandbirthweight≥1800g)withprecocious metabolic,clinicalandelectroencephalographic(EEG)signsofhypoxic-ischemicencephalopathywillbe randomized,accordingtotheirEEGpattern,toreceivetopiramateaddedtostandardtreatmentwithmoderate hypothermiaorstandardtreatmentalone.Topiramatewillbeadministeredat10mg/kgonceadayforthefirst 3daysoflife.Topiramateconcentrationswillbemeasuredonserialdriedbloodspots.64participant
